PreS1Ag与HBcAg检测在诊断乙肝病毒复制的意义

目的分析乙型肝炎病毒（HBV）前S1抗原（PreS1Ag）、核心抗原（HBcAg）与HBV DNA之间的关系,探讨对判断乙肝病毒的复制与指导乙肝的治疗的意义。方法收集门诊和住院乙型病毒性肝炎患者血清标本435例,检测血清乙肝标志物（两对半）、PreS1Ag、HBcAg及HBV DNA,统计分析其中的HBV DNA≥103拷贝/ml（HBV DNA阳性）的393例。结果在HBV DNA阳性393份标本中PreS1Ag、HBcAg和HBeAg表达阳性率分别为83.7%（329/393）,44.3%（174/393）与52.5%（205/393）。PreS1Ag的阳性表达率最高,与HBcAg和HBeAg的表达阳性率差异有统计学意义（P〈0.05）。而HBcAg和HBeAg的表达阳性率无统计学差异（P〉0.05）,PreS1Ag阳性标本中的,HBcAg阳性和阴性百分率的差异有统计学意义（P〈0.05）。结论检测PreS1Ag和HBcAg与HBV DNA关系密切,反映乙肝病毒复制状况。建议在传统两对半的基础上联合检测PreS1Ag和HBcAg。

Significane of combined detection of preS1Ag and HBcAg in analysis of HBV replication

Aim To investigate the correlation of combined detection of both HBV PreSl antigen（preS1Ag）and HBV core antigen（HBcAg）with HBV DNA,so as to analyze its significance of replication of HBV and the treatment of Hepatitis B.Methods There 435 HBsAg positive samples were collected from inpatients and outpatients,then hepatitis B markers,preS1Ag and HBcAg were tested by ELISA and HBV DNA was detected by quantitative real-time PCR（FQ-PCR）.Finally 393 HBV DNA positive samples with HBV DNA≥103copies/ml were analyzed.Results In 393 HBV DNA positive samples,the positive rates of PreS1Ag,HBcAg and HBeAg were 83.7%（329/393）,44.3%（174/393）and 52.5%（205/393）,respectively.The positive rate of PreS1Ag was significant higher than that of both HBcAg and HBeAg（P0.05）.The positive rate was insignificant between HBcAg and HBeAg（P0.05）.Conclusion The correlation of expression rates of PreS1Ag and HBcAg with load of HBV DNA can reflex status of HBV infection and replication.Thus joint detection of the preS1Ag and HBcAg is indicated for clinical analysis.
