带血管神经蒂的示指背侧岛状皮瓣重建拇指感觉功能

为拇指腹软组织缺损重建感觉功能寻找新的可靠供区。解剖１１只成人冰冻尸体手，经动脉插管于桡动脉腕背支灌注红色乳胶，显微镜下解剖观察示指背侧动脉、静脉、神经来源、分支及吻合情况。并将这一研究临床应用２０例。示指背侧皮肤恒定由第１掌背动脉供血，静脉回流以其伴行静脉及掌背静脉为主，神经由桡神经浅支三级分支支配。随访０．５～１年，拇指腹两点辨别觉为３．１±０．２ｍｍ。可设计以第１掌背动脉及桡神经浅支为蒂的示指背侧感觉皮瓣重建拇指腹感觉功能。

INNERVATED DORSAL FLAP OF INDEX FINGER FOR SENSORY RECONSTRUCTION OF THUMB

To find new innervated and vascularized island flap for reconstruction of various skin defects of the volar side of thumb.11 fresh cadavar hands were injected with a red dye and dissected to examine the vascular and neural supplies of the dorsal of index finger under microscope.This flap was used in 20 patients for sensory reconstruction of the thumb.Innervation of the flap was provided by the third branch of the superficial radial nerve.This flap could be designed island flap transfer for reconstruction of various skin defects of the palmar side of the thumb.Sensory reinnervated was excellent.
