子宫动脉化疗栓塞术联合刮宫术治疗胎盘植入的临床分析

目的探讨子宫动脉化疗栓塞术联合刮宫术治疗胎盘植入的临床疗效。方法 15例经彩超或病理证实为胎盘植入的患者,采用Seldinger技术穿刺成功后,将导管分别选择至双侧子宫动脉,注入甲氨蝶呤共50mg,后用明胶海绵颗粒栓塞子宫动脉。并在术后5-6天在B超引导下行刮宫术。结果 15例患者均插管成功,无一例发生严重并发症,均成功保留子宫。术后1月、3月、6月检查E2、FSH、LH均正常。结论子宫动脉化疗栓塞术联合刮宫治疗胎盘植入是一种安全、有效、微创的治疗方法,值得临床进一步推广。

The clinical effect of uterine artery chemotherapy and embolization combined with curettage for placenta increta

Objective：To evaluate the efficacy of uterine artery chemotherapy and embolization combined with curettage for placenta increta.Methods：Fifteen cases with placenta accrete were diagnosed by B-ultrasound or pathology.We used the skill of Seldinger to place the catheter into the uterine arteries,then injected MTX 50mg from catheter,and used the gelfoam sponge particles to embolize the arteries.After the UACE,curettages were performed 5-6 days later under B-ultrasound guided.Results：The operation was successful in all cases.No severe complication occurred and all retain uterus.Follicle stimulating hormone（FSH）,luteinizing hormone（LH） and estriol（E2） were measured before and after the curettage 1,3,6 month.Conclusion：UACE combined with curettage is a safe,effectively and minimally invasive method.It deserves further recommended.
