糖维胶囊对2型糖尿病患者脂质代谢血管内皮功能及微循环状态的影响

摘    要:目的:探讨糖维胶囊对2型糖尿病(T2DM)患者脂质代谢、血管内皮功能及微循环状态的影响.方法:将200例T2DM患者随机分为对照组(100例)和观察组(100例),对照组给予口服降糖药或胰岛素治疗,观察组在对照组用药上加用糖维胶囊,两组疗程均为12周.比较两组治疗前后糖脂代谢水平、血管内皮功能及微循环状态.结果:观察组治疗后胰岛素抵抗(IR)、总胆固醇(TC)、甘油三酯(TG)、低密度值脂蛋白胆固醇(LDL-C)水平低于对照组,高密度脂蛋白胆固醇(HDL-C)水平高于对照组,差异有统计学意义(P<0.05);两组治疗后空腹血糖(FBG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA1c)比较差异无统计学意义(P>0.05);观察组治疗后一氧化氮(NO)、内皮依赖性血管舒张功能(FMD)高于对照组,血浆内皮素-1(ET-1)低于对照组,差异有统计学意义(P<0.05),观察组治疗后输入枝管径、输出枝管径、襻顶血管直径大于对照组,管襻畸形数少于对照组,差异有统计学意义(P<0.05).结论:与常规降糖治疗相比,糖维胶囊联合常规降糖治疗T2DM在调节脂质代谢异常和血管内皮功能、改善微循环障碍上更具优势.

Effect of Tangwei Capsule on Lipid Metabolism,Vascular Endothelial Function and Microcirculation in Patients with Type 2 Diabetes Mellitus
Abstract:Objective:To investigate the effect of Tangwei capsule on lipid metabolism, vascular endo-thelial function and microcirculation in patients with type 2 diabetes mellitus ( T2DM) . Methods: A total of 200 patients with T2DM were randomly divided into the control group (100 cases) and the observation group (100 cases). The control group were treated with oral hypoglycemic agents or insulin, and the observation group were additionally treated with Tangwei capsule. Both groups were treated for 12 weeks. The levels of glu-cose and lipid metabolism, vascular endothelial function and microcirculation were compared between the two groups before and after treatment. Results:After the treatment, the insulin resistance ( IR) , total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) levels in the observation group were lower than those in the control group, while the high-density lipoprotein cholesterol ( HDL-C) level was higher than that in the control group ( P <0.05) . There were no significant differences in fasting blood glucose (FBG), postprandial 2h plasma glucose (2hPG) and glycosylated hemoglobin (HbA1c) between the two groups after treatment ( P >0.05) . After treatment, the nitric oxide ( NO) and endothelium dependent vasodi-lation (FMD) were higher, and plasma endothelin -1 (ET-1) was lower in the obsercation group than the control group ( P < 0.05) . After treatment, the diameters of afferent loop, efferent loop and top of loop were larger in the observation group than the control group, and the number of tube loop malformation was smaller than that in the control group ( P < 0.05) . Conclusion:Compared with routine hypoglycemic therapy, Tang-wei capsule combined with routine hypoglycemic therapy has more advantages in regulating abnormal lipid me-tabolism and vascular endothelial function and relieving microcirculation disturbance in patients with T2DM.
