GP73在肝癌早期诊断中的应用价值

【目的】探讨高尔基体蛋白73（GP73）作为一个原发性肝癌早期诊断的血清标志物的价值。【方法】采用酶联免疫吸附试验定量测定103例原发性肝癌、49例乙型病毒肝炎患者和50名健康人的血清GP73的水平。【结果】肝癌组中血清GP73的水平显著高于肝炎纽及健康对照组（P〈0．05）。通过ROC曲线确定诊断肝癌的GP73临界值为】13．2μg／L，和甲胎蛋白（AFP）的临界值为13．3μg／L时，GP73和AFP单项检测的敏感度分别为68．0%和56．3％，特异度分别为94．8％和91．8％，两者联合检测的敏感度为80．6％，特异性为89．9％。在血清AFP浓度低于7μg／L的肝癌患者中，有52％（17／33）的患者血清GP73浓度高于临界值。【结论]GP73高表达于肝癌患者的血清中，可能成为肝癌早期诊断的一个更好的血清标志物，与AFP联合检测可有效的提高肝癌诊断的敏感度，特别有助于APF阴性肝癌患者的诊断。

Value of GP73 in the Early Diagnosis of Liver Carcinoma

Objective] To explore the value of GP73 as a serum marker of the early diagnosis of primary hepatic earcinoma（PHC）. Methods] Enzyme-linked immunosorbent assay（ELISA） was used to quantitatively detect the serum level of GP73 in 103 cases of PHC, 49 cases of hepatitis and 50 cases of health people. IRe- suits] The serum level of GP73 in PHC was significantly higher（ P 〈0.05） than that in healthy individuals and patients with HBV（ P d0.05）. The critical value of GV73 and AFP were 113.2 μg/L and 13.3 μg/L determined by ROC curves. The sensitivity of GP73 and AFP was 68.0% and 56.3%, and the specificity was 94.8% and 91.8%, respectively. The sensitivity and specificity of the combination of GP73 and AFP were 80.6% and 89.9%, respectively. Moreover, serum GP73 level was higher than the critical value in 52%（17/ 33） of PHC patients with serum AFP less than 7μg/L. Conclusion] GP73 level is high in serum of patients with PHC, and may be a good serum marker of the early diagnosis of PHC. The detection of GP73 combined with AFP can effectively improve the sensitivity of the diagnosis of PHC, and especially contribute to the diagnosis of PHC patients with AFP-negative.
