觉醒型进展性卒中九例

目的对觉醒型进展性卒中的临床特点进行分析。方法对9例符合觉醒型进展性卒中定义的患者临床资料包括性别、危险因素、临床表现、影像学特点、治疗及预后进行描述。结果 9例符合觉醒型进展性卒中包括(1)男性5人,女性4人;(2)高血压病8人,2型糖尿病7人;(3)均以偏身肢体无力为主要表现,伴有失语患者2人,伴有偏身麻木3人,伴吞咽障碍2人;(4)MRA示颈内动脉中-重狭窄2人,大脑中动脉中-重度狭窄4人;(5)6例病人于夜间加重并及时调整药物后5例患者预后较好,1例预后差,3例晨起发现加重者,虽进行药物调整,但预后仍较差。结论对可能是觉醒型进展性卒中患者及时发现病情加重并治疗,对改善预后至关重要。

Clinical analysis of wake-up progressive strokes:9 cases report

Objective To investigate the clinical characteristics of wake-up progressive strokes.Methods The clinical data of 9 cases with wake-up progressive strokes including clinical findings,imaging changed,and treatment et al were analyzed retrospectively.Results 9 cases include:(1) 5 male and 4 female;(2) 8 hypertension,7 diabetes mellitus;(3) The main clinical finding is hemiparalysis,2 cases with aphasia,3 cases with hemianesthesia and 2 with dysphgia;(4) 2 cases with moderate/severe internal carotid artery stenosis,4 cases with moderate/severe middle cerebral artery stenosis;(4)6 patients were adjusted the drugs timely in night,5 patients had favourable prognosis,1 patient had a bad prognosis;3 patients,who had progress stroke in night and were found the next day,had bad prognosis.Conclusion If the patients with wake-up progressive strokes are found and treated timely,they shall have favourable prognosis.
