交锁髓内钉在创伤性下肢长管状骨骨折治疗中的临床应用

目的探讨交锁髓内钉治疗创伤性下肢长管状骨骨折患者的效果。方法选取2018年3月至2019年5月于本院接受手术治疗的106例创伤性下肢长管状骨骨折患者,随机分为两组,每组53例。对比组患者给予钢板内固定术,研究组患者给予交锁髓内钉固定治疗。观察比较两组治疗效果、骨愈合情况、并发症发生率及术后各项指标。结果研究组治疗优良率为98.11%,明显高于对比组的86.79%(P<0.05)。研究组骨性愈合率显著高于对比组(P<0.05),畸形愈合率明显低于对比组(P<0.05),两组患者骨折不愈合率及延迟愈合率比较差异无统计学意义。研究组术后关节功能障碍、髓内钉或者钢板断裂等并发症总发生率为9.43%,明显低于对比组的20.75%(P<0.05)。研究组患者肢体肿胀消退时间、骨折愈合时间、住院时间均短于对比组,各项指标组间比较差异均有统计学意义(P<0.05)。结论应用交锁髓内钉治疗创伤性下肢长管状骨骨折患者,可提高骨性愈合率,且手术安全性更高,有助于降低关节功能障碍等相关并发症。

Clinical application of interlocking intramedullary nail in the treatment of traumatic long bone fracture of lower extremity

Objective To investigate the effect of interlocking intramedullary nails in the treatment of traumatic fractures of long tubular bones of the lower extremities.Methods A total of 106 patients with traumatic long tubular bone fractures of the lower limbs who underwent surgical treatment in our hospital from March 2018 to May 2019 were randomly divided into two groups,with 53 cases in each group.Patients in the control group were treated with plate internal fixation,and patients in the study group were treated with interlocking intramedullary nails.Observe and compare the treatment effect,bone healing,complication rate and postoperative indicators between the two groups.Results The excellent and good treatment rate in the study group was 98.11%,which was significantly higher than the 86.79%in the control group(P<0.05).The bone union rate of the study group was significantly higher than that of the control group(P<0.05),and the rate of malunion was significantly lower than that of the control group(P<0.05).There was no significant difference in the rates of nonunion and delayed union between the two groups.The total incidence of postoperative complications such as joint dysfunction,intramedullary nail or plate breakage in the study group was 9.43%,which was significantly lower than 20.75%in the control group(P<0.05).The patients in the study group had shorter limb swelling subsidence time,fracture healing time,and hospital stay than the comparison group,and there was statistically significant difference among the index groups(P<0.05).Conclusion The use of interlocking intramedullary nails for the treatment of traumatic fractures of long tubular bones of the lower extremities can improve the bone healing rate,and the surgical safety is higher,and it is helpful to reduce joint dysfunction and other related complications.
