股骨上下端粉碎性骨折的治疗

目的:探讨股骨上下端粉碎性骨折治疗方法及疗效。方法:自2005年1月～2008年1月对38例患者,其中股骨上端骨折19例,股骨下端骨折19例,采用切开复位内固定DHS 12例,解剖钢板10例,DCS 7例,髁支持钢板9例治疗。结果:本组经6个月～24个月,平均16个月随访,35例骨性愈合,3例仍在随访中,优良率87.5%。结论:股骨上下端粉碎性骨折,采用DHS、DCS及股骨上段解剖钢板、股骨下段髁支持钢板治疗后,固定牢靠,患者能早期进行髋膝关节功能锻炼,减少骨质疏松,促进骨折愈合。

The Treatment of Comminuted fracture of Proximal or Distal Femur

Objective:To discuss the treatment methods and effects of comminuted fracture of proximal or distal femur.Methods: From January 2005 to January 2008,38 cases were treated which included 19 cases with comminuted fracture of proximal femur,19 distal femur.12 cases were operated by dynamic hip screw(DHS),10 by anatomical steel plate,7 by dynamic candyle screw,9 by supportable candyle plate.Results: They had been followed up for six to twenty-four months,an average of 16 months.35 cases had bony union,3 cases were followed up,the excellent rate was 87.5%.Conclusion DHS and anatomical steel plate of proximal femur might be an excellent technique for comminuted fracture of proximal femur,DCS and supportable condyle screw for distal femur,which provide stable fixation,so that early mobilization and weight-bearing should be allowed,reduce osteoporosis and accelerate bony union.
