宣肺止喘汤在婴幼儿毛细支气管炎中的应用研究

目的：观察中药宣肺止喘汤对婴幼儿毛细支气管炎的临床作用。方法：将符合标准的80例患者按随机数字表法平均分为观察组和对照组。两组患者均采取西医常规处理,以抗生素、支气管扩张剂、抗过敏药及祛痰剂进行对症支持治疗。观察组加用中药宣肺止喘汤（炙麻黄、杏仁、生石膏、连翘、黄芩、鱼腥草、蒲公英、炒葶苈子、炒苏子、炒莱菔子、地龙、浙贝母、神曲、焦山楂、生甘草）随证加减治疗。观察患儿临床症状（咳嗽、憋喘、发热）、体征（哮鸣音、湿啰音）消失时间。记录患儿治疗后24 h、48 h和72 h的CS评分。于治疗结束3个月后随访,记录复发率。结果：观察组和对照组显效率分别为72.50%、45.00%,总有效率分别为97.50%、87.50%,复发率分别为7.69%、18.42%。组间比较,差异均有统计学意义（P〈0.05）。观察组患儿治疗后48 h和72 h的CS评分均少于对照组。组间比较,差异均有统计学意义（P〈0.05）。观察组患儿各项主要临床症状和体征的缓解时间均少于对照组。组间比较,差异均有统计学意义（P〈0.05）。治疗后组间比较,差异均有统计学意义（P〈0.05）。结论：中药宣肺止喘汤联合西医对症支持治疗,可有效改善毛细支气管炎患儿的临床症状、体征和肺功能,缩短疗程,降低近期复发率。

Xuanfei Zhichuan Decoction in Infants with Bronchiolitis in Applied Research

Objective ： To observe the effect of Xuanfei Zhichuan Decoction on clinical effect of infants with bronchiolitis. Methods ： 80 cases of the eligible patients were randomly divided into observation group and control group. 2 groups＇ patients took routine western medical treatment, antibiotics, bronchodilators, anti allergy and supportive treatment. The observation group added traditional Chinese medicine Xuanfei Zhichuan Decoction （ephedra, almond, piaster, forsythia, scutellaria root, Houttuynia, dandelion, fried pepperweed seed, fried perilla seed, semen Raphani, earthworm, Zhejiang Fritillaria, the Divine Comedy, Jiao Shanzha, born licorice） changing in treating. We observed the clinical symptoms （cough, wheeze, fever） , symptoms （wheezing, wet rales） disappeared time and recorded CS score 24 h, 48 h and 72 h after treatment. In the 3 months of follow - up after discharge from hospital, pulmonary function test was made and the recurrence rate was recorded. Results ： The observation group and the control group＇s significant efficiencies were 72.50% and 45% and the total effective rates were 97.50% and 87.50%. The recurrence rates were 7.69%, 18.42%. Comparison between groups was statistically significant （P 〈 0.05 ）. The observation group＇s scores 48 h and 72 h after treatment of CS were lower than those of the control group. Comparison between groups was statistically significant （ P 〈 O. 05）. The observation group＇s main clinical symptoms and signs of remission time were lower than those of the control group. Comparison between groups was statistically significant （ P 〈 0.05 ）. Conclusion： The Traditional Chinese medi- cine Xuanfei Zhichuan Decoction combined with western medicine treatment can effectively improve the bronchiolitis ehildren＇s clinical symptoms, signs and pulmonary function, shorten the course of treatment and reduce recurrence rate.
