摘    要:目的探讨导管消融治疗房性早搏(PACs)的临床疗效。方法 10例PACs病人,Holter提示24 h PACs17 957.2±4 532个,4例伴有短阵房性心动过速,根据心内激动顺序,初步确定感兴趣区域,应用激动标测法进行标测,在相对最提前部位消融,采用标准为提前≥30 ms。结果手术时间1.6±0.6 h,X线曝光时间21.6±5.4min。每例病人消融2.8±0.8(2~5)次。术后第3日Holter检查,5例病人PACs完全消失,3例病人PACs在2~12个之间,2例病人PACs分别为307及204个,此2例术前均为双源性PACs,残余均为另外一种形态的PACs。随访9.3±4.2个月,无1例复发。结论导管消融治疗PACs是安全和有效的。

Abstract:Objective The purpose of the study is to assesss the efficacy and safety of catheter ablation in premature atrial contractions(PACs)by electrophysiological mapping.Methods The study included 10 patients with symptomatic drug refractory PACs,four of which combined with short paroxysmal atrial tachycardia.After determining the target area according to the active sequence of the intracardia recording,mapping was performed by active mapping.Ablation was performed at the relative earliest site which was 30 ms earlier than reference point.Results The mean procedure time was 1.6±0.6 hours and the mean X-ray exposure time was 21.6±5.4 minutes.The mean number of energy application was 2.8±0.8 times.When examined in the third day after the procedure,none of PACs was found in 5 patients.There were 2~12 PACs in 3 patients.There were PACs of another morphology in 2 patients.There was no complication.After 9.3±4.2 months of follow-up,there was no PAC recurred.Conclusion Catheter ablation in PACs by electrophysiological mapping is effective and safe.
