脑卒中后抑郁状态临床分析

目的 ]观察脑卒中后抑郁状态发生率与病损部位及神经功能缺损程度的关系以及盐酸舍曲林 (左洛复 )治疗此症的疗效。 方法 ]采用汉密尔顿抑郁量表了解卒中后抑郁症状、抑郁发生率及抑郁程度 ,神经功能缺损程度量表评定神经功能缺损程度 ,头CT或MRI检查病损部位 ,对有抑郁症状的患者给予盐酸舍曲林治疗。 结果 ]卒中后抑郁状态与疾病严重程度呈正相关 (P <0 .0 1 ) ,且大脑皮质、丘脑、脑干卒中患者抑郁发生率高 (P <0 .0 1 ) ,盐酸舍曲林治疗有效。 结论 ]卒中后抑郁状态与疾病严重程度呈正相关 ,与脑卒中病损部位有关 ,盐酸舍曲林对卒中后抑郁状态治疗有效。

Clinical analysis of treatment effect on post-stroke depression

Objective]To study the relationship of post-stroke depression with the location of lesion and the severity of neurological impairment, and observe the contribution of sertraline hydrochloride to depression.Methods] Hamilton rating scale for depression (HAMD) was performed in 326 stroke patients in the Fourth Affiliated Hospital of Dalian Medical University, including 68 cases post-stroke depression. Thirty-eight patients with post-stroke depression as treatment group orally were treated with sertraline hydrochloride 50mg once a day following routine drug therapy, and 30 patients as controls only received routine drug therapy, lasting 6 weeks. Correlation of depression among types of stroke, focal sites and the severity of neurological impairment were observed, and HAMD was used to evaluate the curative effect of oral sertraline hydrochloride treatment for depressive patients. Results] There was a significant correlation between the degree of neurological impairment and the incidence of post-stroke depression ( P <0.01). The incidence of depression in patients with acute cortex, thalamus and brain stem stroke is higher than sub-cortex stroke ( P <0.01). Sertraline hydrochloride has a good effect on post-stroke depression. Conclusion] Post-stroke depression has a significant correlation with the local site and the degree of neurological impairment, and it can effectively be improved sertraline hydrochloride.
