某院4科室围术期预防性应用抗菌药物对比分析

目的:了解抗菌药物专项整治活动前后某院4科室围术期预防性应用抗菌药物情况。方法:对某院4科室Ⅰ类切口手术患者抗菌药物临床应用专项整治活动前(2011年1—4月,240例)和整治活动后(2013年1—4月,240例)围术期抗菌药物的使用情况进行统计和对照研究。结果:抗菌药物临床应用专项整治活动后,Ⅰ类切口手术抗菌药物使用率和无指征预防性用药率分别由整治活动前的100%、63.75%分别降至7.92%、3.75%;药物选择不合理率由整治活动前的85.42%降至10.83%;术前给药时间和术后用药疗程不合理率均由整治活动前的83.75%、92.92%分别降至25.00%、22.92%;平均抗菌药物费用由整治活动前的661.38元降至18.01元;上述指标整治前后两两比较,差异均有统计学意义(P<0.05)。结论:抗菌药物专项整治活动使该院抗菌药物临床应用更加合理。

Comparatwe Analysis of Perioperative Antibiotic Use in Four Clinical Departments of A Hospital

OBJECTIVE：To evaluate the perioperative use of prophylactic antibiotics in 4 clinical departments of a hospital before and after implementation of Special Rectification Activities on Clinical Antibiotic Clinical Use.METHODS：The perioperative use of prophylactic antibiotics in 4 clinical departments of a hospital before and after implementation of Special Rectification Activities on Clinical Antibiotic Clinical Use（240 cases before intervention during January-April period of 2011 versus 240 cases after intervention during January-April period of 2013） was analyzed contrastively.RESULTS：The rates of antibiotic use,non-indicated preventive antibiotic use,irrational choice of antibiotics,irrational preoperative timing and post-operative duration of antibiotic use decreased from 100％,63.75％,85.42％,83.75％ and 92.92％ respectively before intervention to 7.92％,3.75％,10.83％,25.00％ and 22.92％ respectively after intervention,and the average cost of antibiotics decreased from 661.38 yuan before intervention to 18.01 yuan after intervention,with significant differences noted in all indicators before and after intervention （P＜0.05）.CONCLUSIONS：The Special Rectification Activities on Clinical Antibiotic Clinical Use contributed more rational clinical use of antibiotics in a hospital.
