腹腔镜手术联合GnRH激动剂治疗重度子宫内膜异位症120例临床分析

目的观察腹腔镜手术联合GnRH激动剂治疗重度子宫内膜异位症的疗效。方法 2004年7月～2008年7月我院对120例重度子宫内膜异位症采用腹腔镜手术治疗宫内膜异位病灶,术后给予达菲林或诺雷德治疗6个月。结果 120例均行腹腔镜卵巢囊肿剥除术+盆腔粘连松解术+盆腔子宫内膜异位病灶电灼术,其中3例行宫骶韧带切断术,6例行双侧圆韧带悬吊术。120例随访6～60个月,平均26个月,复发率8.3%(10/120),73例合并不孕治疗后1年自然妊娠率42.5%(31/73)。结论腹腔镜手术联合GnRH激动剂治疗重度子宫内膜异位症临床效果满意。

Laparoscopic Surgery Combined with GnRH Agonist for Severe Endometriosis: Analysis on 120 Cases

Objective To evaluate the efficacy of laparoscopy combined with GnRH agonist in the treatment of severe endometriosis. Methods From July 2004 to July 2008,we performed laparoscopy on 120 patients with severe endometriosis,after the operation,triptorelin acetate or Zoladex were administered for 6 months. Results All of the cases received laparoscopic oophorocystectomy＋ lysis for pelvic adhesion ＋ coagulation of pelvic endometriosis. Three patients underwent uterosacral ligament amputation,and six patients received bilateral round ligament suspension. The 120 patients were followed up for 6 to 60 months with a mean of 26 months,10 of them （8.3%） showed recurrent lesions during the period. Of the cases,73 patients received treatments for infertility,31 of them had natural pregnancy in one year （42.5%）. Conclusion Laparoscopy combined with GnRH agonist is an effective method for the treatment of severe endometriosis.
