摘    要:目的探讨临床诊断用ABR仪通过加载SmartScreen程序进行新生儿蜗后病变筛查的可行性。方法2006年6月至2009年10月在解放军第309医院和解放军第316医院妇产科出生的2864例新生儿中的550例在出生后42d后接受AABR筛查,用美国智听公司诊断用ABR仪加载SmartScreener测试程序进行。AABR采用35dBnHL的短声刺激,刺激率为19.3次/s,叠加500—2000次,在标准隔声室内进行测试。对于筛查不通过者转诊至上级听力诊断中心进行听力评估。结果550例新生儿中535例通过了初筛,筛查通过率97.27%(535/550),所有测试未通过者转往北京市指定的听力筛查中心,转诊百分比5.24‰(15/2864),诊断听力正常12例,确诊感音神经性聋、先天性小耳畸形和唇腭裂各1例次,占同期出生新生儿的1.05‰(3/2864)。AABR筛查通过的535例婴儿,随访半年以上,听力发育未见明显异常。结论诊断用ABR仪加载SmartScree—net测试程序不需要添加额外设备,利用产妇产后42d到医院复查时对婴儿进行筛查,如发现筛查异常还可立即进行诊断性ABR检查,具有一定的推广应用价值。

Newborn Hearing Screening by Diagnostic Auditory Brain Response

Abstract:Objective To discuss the feasibility of applying diagnostic auditory brain response with Smart Screen Program in newborn hearing screening. Methods AABR was recorded with Smart Screen Program in 550 neonates from June of 2006 to October of 2009. All the neonates were followed for at least 6 months and the development of hearing was focused. Results Totally 15 infants failed to pass the AABR screening and the refer rate was 5.24‰(15/2864). Of the 15 referred infants, three had abnormal hearing after hearing diagnosis including 1 sensorineural hearing loss, 1 cleft palate and 1 malformed ear. The follow - up results revealed that all the 535 infants passing the screening test had normal hearing till then. Conclusion Diagnostic auditory brain response with Smart Screen is simple to do the AABR screening without extra equipment. And infants can be screened 42 days after birth when their mothers go to the hospital to have postpartum check. Once abnormity found, ABR can be recorded immediately. For its convenience and practicality, the diagnostic auditory brain response with Smart Screen Program might be valuable to popularize AABR in newborn hearing screening in China.
