脑静脉窦血栓形成15例临床分析

目的探讨脑静脉窦血栓形成(CVST)的临床特点、影像学表现和治疗方法。方法对15例脑静脉窦血栓形成患者的临床资料进行回顾性分析。结果本组中12例(80.0%)患者临床表现以颅高压症状如头痛、恶心呕吐为主,8例(53.3%)患者存在局灶性脑损伤症状。影像学检查可发现形式多样的脑实质病灶,并可显示受累静脉窦。治疗方法包括抗凝治疗、病因治疗、对症支持治疗等。治疗后临床显著改善率80.0%(12例),病死率6.7%(1例)。结论 CVST临床表现无特异性,MR结合MRV检查可作为早期诊断的首选方法。抗凝治疗是CVST的主要治疗方法,该病经治疗后预后较好。

Clinical characteristic of fifteen patients with cerebral venous sinus thrombosis

Objective To investigate the clinical characteristic,image findings and treatment of patients with cerebral venous sinus thrombosis(CVST).Methods To retrospectively analyze the clinical data of 15 patients with CVST that comes from Shenzhen municipal second peoples hospital.Results 12 patients(80.0%)with clinical manifestations of intracranial hypertension such as headache,nausea and vomiting mainly,8 patients(53.3%)with symptoms of focal brain damage.Image studies showed the various forms of brain parenchymal lesions,and sinus involvement.The treatments include anticoagulant therapy,etiology therapy,symptomatic and supportive treatment.After treatment,clinically significant improvement rate was 80.0%(12 patients),mortality rate of 6.7%(1 patient).Conclusion Clinical manifestations of CVST are nonspecific.MR and MRV examinations can be used as the preferred method for early diagnosis.Anticoagulant therapy is the main treatment of CVST which has good prognosis after treatment.
