脑梗死患者抑郁严重程度与睡眠质量的相关性

目的探讨脑梗死患者抑郁严重程度与睡眠质量的相关性,为临床诊治提供依据。方法将93例脑梗死患者按抑郁严重程度分为3组,轻度抑郁组30例,中度抑郁组38例,重度抑郁组25例。采用汉密顿抑郁量表、匹兹堡睡眠质量指数量表分别评定抑郁严重程度及睡眠质量,采用Pearson相关分析探讨抑郁与睡眠质量的相关性。结果3组匹兹堡睡眠质量指数量表总分及睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、日间功能障碍因子分比较差异有统计学意义(P<0.01),其中重度抑郁组>中度抑郁组>轻度抑郁组,组间两两比较差异有统计学意义(P<0.01);3组催眠药物因子分比较差异无统计学意义(P>0.05)。Pearson相关分析显示,脑梗死患者汉密顿抑郁量表评分与匹兹堡睡眠质量指数量表总分及睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、日间功能障碍因子分呈显著正相关(P<0.01)。结论脑梗死患者抑郁严重程度与睡眠质量密切相关,临床应重视对患者睡眠质量的评估及干预,以改善患者抑郁程度及预后。

Correlation between depression and sleep quality in patients with cerebral infarction

Objective To explore the relationship between depression and sleep quality in patients with cerebral infarction,to provide a basis for clinical diagnosis and treatment.Methods A total of 93 patients with cerebral infarction were divided into 3 groups according to the severity of depression,30 cases in the mild depression group,38 cases in the moderate depression group,and 25 cases in the severe depression group.The Hamilton depression scale(HAMD)and the Pittsburgh sleep quality index(PSQI)were used to assess the severity of depression and sleep quality,respectively.The Pearson correlation analysis was used to explore the correlation between depression and sleep quality.Results There were statistical significant differences in PSQI total score and the factor score of sleep quality,time to fall asleep,sleep time,sleep efficiency,sleep disturbance,daytime dysfunction among the three groups.Among them,the severe depression group>the moderate depression group>the mild depression group,the difference between the two groups was statistically significant(P<0.01).There was no statistical significant difference in the hypnotic factor score among the 3 groups(P>0.05).Pearson correlation analysis showed that the HAMD score of patients with cerebral infarction was significantly positively correlated with the total PSQI score and sleep quality,time to fall asleep,sleep time,sleep efficiency,sleep disturbance,and daytime dysfunction factor scores(P<0.01).Conclusions The depression of patients with cerebral infarction is closely related to the quality of sleep.In clinical practice,the evaluation and intervention of the sleep quality of patients should be emphasized to improve the degree of depression and prognosis of patients.
