桡骨远端不稳定骨折不同术式的临床应用

目的比较桡骨远端不稳定骨折应用不同手术方式的疗效和临床价值。方法对桡骨远端不稳定骨折62例,根据其损伤类型分别采用普通T型钢板固定21例、锁钉加压钢板(LCP)固定7例、单纯克氏针固定5例、克氏针 外固定支架固定20例、T型钢板 外固定支架固定9例。植骨者36例,手术均在伤后10天内进行。结果术后随访6~24个月,根据Dienst标准,各组的优良率分别为:T型钢板固定90.5%,LCP钢板固定85.7%,单纯克氏针固定100.0%,克氏针 外固定支架固定95.0%,T型钢板 外固定支架固定组88.9%。结论桡骨远端不稳定骨折根据其损伤类型,采用相对应的手术方法均可取得良好的疗效。对严重的桡骨远端不稳定骨折及开放性骨折,外固定支架固定优于其他组。

Clinical application of different internal fixation methods to distal radius unstable fractures

Objective To compare the curative effect between different operation methods for distal radius unstable fracture.Methods We had summarized 62 cases of radius distant unstable fracture including 21 cases fixed with ordinary T plate,21 fixed with LCP,5 fixed with pure Kirschner wire,20 fixed with Kirschner wire plus external fixation frame and 9 fixed with T plate plus external fixation frame.36 cases were transplanted with bone grafts and all operations were performed within 10 days after injury.Results During postoperative follow up for 6-24 mouths,according to Dienst standard,the good recovery rates were as follows: 90.4% in ordinary T plate group,85.7% in LCP group,100% in pure Kirschner wire group,95% in Kirschner wire plus external fixation frame group and 88.9% in T plate plus external fixation frame.Conclusion According to the injury types of distal radius unstable fracture,the corresponding operating method is used for good curative effect.However,for the severe or unstable open distal radius,the external fixation frame is better than other fixed methods.
