短节段椎弓根螺钉内固定治疗脊柱胸腰段爆裂性骨折对恢复脊柱高度及功能的疗效

目的探讨短节段椎弓根螺钉内固定治疗脊柱胸腰段爆裂性骨折对恢复脊柱高度及功能的临床效果。方法选取本院2010年4月～2011年10月收诊的47例脊柱胸腰段爆裂性骨折患者为研究对象,均给予短节段椎弓根螺钉内固定治疗,观察术中出血量、手术时间、伤椎高度丢失率、复位率以及神经功能的恢复情况。结果所有患者均顺利完成手术治疗,术中出血量为（116.83±17.92）ml,手术时间为（83.83±15.92）min,受伤脊椎椎体复位率为（85.62±11.04）%,对所有患者随访6～24个月,椎体高度丢失率为（16.83±7.92）%;术后神经恢复情况评估：A级0例,B级2例,C级4例,D级9例,E级32例。结论短节段椎弓根螺钉内固定治疗脊柱胸腰段爆裂性骨折可以很好地恢复脊柱高度和功能,操作简单,安全有效,值得在临床上推广应用。

Effect on short-segment pedicle screw fixation in treatment of thoracolumbar burst on the recovery of vertebral height and functions of fracture

Objective To investigate the effect of short-segment pedicle screw fixation in treatment of thoracolumbar burst on the recovery of spinal height and the function of fracture.Methods 47 cases of thoracolumbar burst fracture patients as the research object from April 2010 to December 2011 in our hospital,were given the treatment of short segment pedicle screw fixation.The intraoperative bleeding volume,operation time,vertebral height loss rate,reduction rate and the recovery of neurological function were observed.Results All patients were completed the operation treatment successfully.The operation bleeding volume was（116.83±17.92） ml,the operation time was（83.83±15.92） min,the injured spine vertebra reduction rate was（85.62 ±11.04）%,after 6-24 months follow-up,the vertebral height loss rate was（16.83±7.92）%.The nerve recovery after operation was evaluated：0 case of grade A,2 cases of grade B,4 cases of grade C,9 cases of grade D,32 cases of grade E.Conclusion Short-segment pedicle screw internal fixation in the treatment of thoracolumbar burst fractures can be very good to restore the spinal height and functions.The operation is simple,safe and effective.It is worth promoting in clinical use.
