高脂血症性急性胰腺炎的临床分析

目的探讨高脂血症(HL)与急性胰腺炎(AP)的关系。方法回顾大竹县红十字医院2000年1月至2009年12月收治的581例急性胰腺炎患者,分析脂肪肝、高脂血症与急性胰腺炎发病的关系。结果在581例AP中有48例高脂血症性急性胰腺炎(8.3%),与其他原因所致的胰腺炎相比,高脂血症性胰腺炎(HLAP)血三酰甘油(TG)升高明显(P<0.05),血总胆固醇(TC)升高不明显;糖尿病和脂肪肝发病率与其他原因所致的胰腺炎相比差异有统计学意义(P<0.05)。结论高三酰甘油血症和胰腺炎关系密切,是胰腺炎的一个独立危险因素,HLAP与其他原因所致的胰腺炎相比,其糖尿病和脂肪肝发病率明显增高。临床上降脂治疗对HLAP很重要。

The Clinical Analysis of Hyperlipidemic Acute Pancreatitis

Objective To investigate the relationship between hyperlipidemia（HL） and acute pancreatitis（AP）.Method By looking back the 581 cases of acute pancreatitis patients admitted from the January of 2000 to the December of 2009 in Dazhu County Red Cross Hospital,we analyzed the relations of the morbidity between the fatty liver,hyperlipemia and acute pancreatitis pathogenesis.Results Among the 581 cases of AP,there are 48 patients having the hyperlipidemia and acute pancreatitis（8.3%）,in comparision with the other patients with pancreatitis caused by other reasons,serum triglyceride（TG） in the patients with the hyperlipidemic pancreatitis（HLAP） increased significantly（P0.05）,serum total cholesterol（TC） didn＇t increase obviously;the differences between the incidence of diabetes and fatty liver and pancreatitis caused by other reasons were of significance for statistics（P0.05）.Conclusion Hypertriglyceridemia and pancreatitis are closely related,which is an independent and riskable factor for pancreatitis.In Comparision with pancreatitis caused by other reasons,the incidence of diabetes and fatty liver in HLAP was significantly higher,so lipid-lowering therapy on the clinical is very important to HLAP.
