β-七叶皂昔钠联合甲泼尼龙减少一氧化碳中毒迟发脑病发病率的相关研究

摘    要:目的探讨β-七叶皂甘钠联合甲泼尼龙减少一氧化碳中毒迟发脑病发病率的相关性。方法选取2018年11月—2019年2月在保定市第二医院治疗的一氧化碳中毒患者120例,回顾性分析,分为β-七叶皂昔钠联合甲泼尼龙组(简称观察组)和β-七叶皂甘钠组(简称对照组),观察两组患者迟发性脑病发病率,检测各组患者血清中Caspase-3、Casepase-9、IL-4、IL-10的变化以及观察两组患者服药后的不良反应。结果对两组患者疗效进行比较发现,观察组患者优良率88.3%(53/60)明显高于对照组患者73.3%(44/60)(P<0.05),迟发性脑病的发病率6.7%(4/60)明显低于对照组患者20.0%(12/60)(P<0.05)。对发生迟发性脑病的患奢进行Caspase-3、Caspase-9、IL-4、IL-10检测发现,其Caspase-3、Caspase-9、IL-4、IL-l0水平明显低于未发病患者;同时对比两种用药出现的不良反应发现差异无统计学意义(P>0.05)。结论β-七叶皂昔钠联合甲泼尼龙可以有效改善一氧化碳中毒患者并发迟发脑病的概率,降低患者体内Caspase-3、Caspase-9、IL-4、IL-10水平,值得临床进一步研究。

Abstract:Objective To explore the efficacy of sodium p-aescinate in reducing the incidence of delayed encephalopathy caused by carbon monoxide poisoning.Methods A retrospective selection of 120 patients with carbon monoxide poisoning treated in our hospital from November 2018 to February 2O19.They were divided into two groups:β-aescin sodium combined with methylprednisolone group(observation group)and β-aescin sodium group(control group).To observe the incidence of delayed encephalopathy in the two groups,to detect the changes of serum Caspase-3、Casepase-9、IL-4、IL-10 in each group and to observe the adverse reactions of the two groups after taking medicine.Results The treatment effectiveness excellent rate of the observation group was 88.3%(53/60),significantly higher than that of the control group(73.3%,44/60,P<0.05).The incidence rate of delayed encephalopathy of the observation group was 6.7%(4/60),significantly lower than that of the control group(20.0%,12/60,P<0.05).The levels of caspase-3,caspase-9,IL-4,and IL-10 of the patients with delayed encephalopatliy were all significantly lower than those of the patients without delayed encephalopathy;and there was no significant difference in the adverse reaction rate between two groups(P>0.05).Conclusion Sodium β-aescinate combined with methylprednisolone effectively reduces the probability of delayed encephalopathy in the patients with carbon monoxide poisoning.
