子宫血管平滑肌瘤5例及文献复习

目的:探讨子宫血管平滑肌瘤的临床表现、病理特征和诊断及鉴别诊断。方法:对5例子宫血管平滑肌瘤的临床表现,病理结果进行研究,并对相关文献进行分析。结果:5例子宫血管平滑肌瘤病例,发病年龄在27～45岁,平均年龄38.8岁。临床表现以月经量增多,经期延长为主要表现,术前均以"子宫肌瘤"为主要诊断。主要病理表现为瘤体由瘤样增生的平滑肌细胞构成,伴有血管增生,瘤细胞呈梭形,核分裂相罕见,瘤细胞围绕血管排列呈漩涡状。随访有一例复发。结论:子宫血管平滑肌瘤无特殊临床表现,早期易误诊为子宫肌瘤。确诊通常依靠病理学诊断。该病存在复发可能,需进行严密随访。

Five cases of uterine angiomyoma

Objective: To explore clinical manifestation,pathological character,diagnosis and differential diagnosis of uterine angiomyoma.Methods:Clinical manifestation,pathological character of 5 uterine angiomyoma cases were observed and analyzed.Results:Five cases aged 27-45 years old and were 38.8 years old in average.They had excessive menstrual blood volume and bradymenorrhea.They were diagnosed as hysteromyoma before undergoing surgery.The main pathological manifestation was tumor body consisting of tumor-like hyperplasia of smooth muscle cell,blood vessel hyperplasia,tumor cell in fusiform,rare mitosis figures,and whirlpool alignment of tumor cell around vessels.One case had relapse.Conclusions: Uterine angiomyoma patients have no special clinical manifestation,and can be easily diagnosed as hysteromyoma.Pathological diagnosis is needed for confirmation.And patients need close follow-up due to possibility of relapse.
