自体外周血干细胞移植治疗恶性血液病的临床研究

目的　探讨自体外周血干细胞移植(APBSCT) 治疗血液恶性肿瘤的疗效。方法　7 例急性白血病,2 例非霍奇金淋巴瘤(NHL) ,采用APBSCT。AL和NHL 在初次完全缓解后经4 ～5 次的骨髓抑制性化疗作体内净化后移植。预处理方案:急性非淋巴细胞性白血病(ANLL) 和NHL 用MAC 方案,急性淋巴细胞性白血病(ALL) 用AEC 方案。结果　所有患者均获造血功能重建,中位随访时间258(68 ～712)天,无移植相关死亡。1 例ANLL及1 例ALL- L3 分别于移植后194 天和49 天复发。结论　初步结果提示APBSCT能延长AL和NHL患者的无病生存期,且具有移植后造血功能恢复快、合并症少等优点

Treatment of malignant hematopathy with APBSCT

Objective To evaluate the curative effect of autologous periphral blood stem cell transplantation (APBSCT) to acute leukemia (AL) and malignant tumor. Methods 7 patients with AL, 2 non-Hodgkin's lymphoma(NHL), 1 late small cell lung cancer(SCLC) were treated with APBSCT. Before the transplantation, patients with AL and NHL were given marrow inhibition chemotherapy for 4-5 times as vivo purification after the first complete remission. The chemotherapy to acute non-lymphocyte leukemia(ANLL) and NHL was MAC( melpholan 140-160mg/m 2, arabinoside cytosine Ara-c 3.0g/m 2, and cyclophsphamide CTX 120mg/kg); to acute lymphocyte leukemia(ALL) was AEC (Ara-c 3.0g/m 2, VP16 1.0g/m 2 and CTX120mg/kg). Results All patients got their hemapoietic function again, medium following 258(68-712) days, there was no death caused by the transplantation, one ANLL relapsed in 194 days, and one ALL-L3 relapsed in 49 day. Conclusion The initial results showed that APBSCT could prolong anosis life of AL, NHL patients, the hemapoietic function recovered quickly, and with fewer complications with this therapy.
