热毒宁注射液治疗呼吸系统感染的循证评价

目的:基于文献分析方法,评价热毒宁注射液用于呼吸系统感染的有效性和安全性,为临床实践与决策者提供循证证据。方法:计算机检索PubMed、the Cochrane Library、中国知网、万方数据库、中国生物医学文献服务系统,纳入热毒宁注射液治疗呼吸系统感染的系统评价/荟萃分析(Meta分析)(干预组为热毒宁注射液单独使用或与其他药物联合应用;对照组为安慰剂或其他阳性药物,单独使用或联合应用其他药物),由2名评价者根据纳入与排除标准筛选文献、提取资料和评价质量后,对证据进行综合梳理与分析。结果:共纳入8篇系统评价/Meta分析。在社区获得性肺炎方面,在常规抗菌药物治疗基础上联合应用热毒宁注射液可明显提高有效率,并明显改善相关临床症状消失时间,差异均具有统计学意义(P<0.05);热毒宁注射液的效果与喜炎平注射液、炎琥宁注射液和痰热清注射液等相当。在治疗上呼吸道感染方面,与利巴韦林、喜炎平注射液、清开灵注射液相比,热毒宁注射液可能具有一定的有效性优势。有限的数据显示,与对照组方案相比,使用热毒宁注射液未增加安全性风险。结论:在常规治疗基础上联合应用热毒宁注射液可提高呼吸系统感染治疗的有效性,与利巴韦林和其他中药注射剂效果基本相当或具有一定的优势。受纳入的文献质量所限,上述结果仍需进一步开展高质量临床研究予以验证。

Evidence Based Evaluation of Reduning Injection in the Treatment of Respiratory Infection

OBJECTIVE:To evaluate the efficacy and safety of Reduning injection in the treatment of respiratory infection based on literature analysis,so as to provide evidence-based evidence for clinical practice and decisionmakers.METHODS:PubMed,the Cochrane Library,CNKI,Wanfang and SinoMed database were systematically retrieved to collect the systematic review/Meta-analysis of Reduning injection in the treatment of respiratory infection(the intervention group was given Reduning injection alone or in combination with other drugs,the control group received placebo or other positive drugs,alone or in combination with other drugs).According to the inclusion and exclusion criteria,two reviewers screened the literature,extracted the data and evaluated the quality.The evidence was comprehensively combed and analyzed.RESULTS:A total of 8 systematic reviews/Meta-analysis were included.In terms of community-acquired pneumonia,the combined application of Reduning injection on the basis of conventional antibacterial treatment can significantly increase the effective rate and significantly shorten the disappearance time of related clinical symptoms,the differences are statistically significant(P<0.05).The effect of Reduning injection was comparable to that of Xiyanping injection,Yanhuning injection and Tanreqing injection.Compared with ribavirin,Xiyanping injection and Qingkailing injection,Reduning injection might have some advantages in the treatment effectiveness of upper respiratory tract infection.Limited data showed that compared with the control group,the application of Reduning injection did not increase the safety risk.CONCLUSIONS:Combined application of Reduning injection on the basis of conventional treatment can improve the effectiveness of the treatment of respiratory infection,which is basically equivalent to or has certain advantages with ribavirin and other traditional Chinese medicine injections.Due to the limited quality of the included literature,the above results still need to be verified by further high-quality clinical studies.
