幽门螺杆菌毒力菌株感染与胃十二指肠疾病的关系

应用间接ELISA法 ,测定 1 674例经胃镜检查患者的血清幽门螺杆菌 (HP)细胞毒素相关基因A(CagA)抗体 ,探讨HP菌型差异与其致胃十二指肠疾病的关系。结果 :疾病组血清CagA抗体检出率为 4 9.70 % ,高于健康组的38% (P <0 .0 5) ;活动期十二指肠溃疡 (DU)的抗体检出率显著高于活动期胃溃疡 (GU)、胃癌 (GC)和慢性胃炎 (CG)(P <0 .0 1 ) ;活动性炎症的抗体检出率显著高于非活动性炎症 (P <0 .0 1 )。随CG炎症程度加重 ,抗体阳性率呈递增性增加 (P <0 .0 5,P <0 .0 1 ) ;GC及各癌前病变抗CagA检出率与健康组比较无差异。提示 :HP产毒菌感染与胃十二指肠粘膜炎症活动及损伤程度有关

Relationship between Specific HP Strain Infection with CagA Seropositive and Gastroduodenal Diseases

In order to assess the relationship between different HP bacteria types and their pathogenicity,investigated 1 674 patients who underwent gastroscopy and 100 blood donors. HP infection was diagnosed by at least 2 positive results from PCR, histology (Giemsa staining), or rapid urease test. Serum CagA IgG antibodies were measured by ELISA. It was found that total prevalence of CagA seropositive was 49.70% in patients and 38% in controls ( P <0.05). The frequency of CagA antibodies were significantly higher in DU cases than that of in GU,GC and CG ones( P <0.01). In active and severe degree of CG, anti CagA presence was more common( P <0.01). However, no difference was found in anti CagA positive rate among GC, CAG, IM, AH and normal groups. These data indicated that specific HP strain with CagA seropositive was associated with the degree of damages and active inflammation on gastroduodenal mucosa.
