摘    要:目的 探索目前国内对肠道菌群与针灸治疗效应及作用机理的相关性研究的状况,为相关领域的科研及临床研究在今后进一步开展提供参考。方法 基于中国期刊全文数据库、维普数据库和万方知识服务平台三大中文数据库以及PubMed英文数据库进行文献检索,时间限定为1999年7月1日-2019年12月1日,筛选符合预定标准的文献建立数据库,运用文献计量学的方法对其进行定量分析。结果 共纳入82篇文献,自2009年开始有针灸治疗与肠道菌群相关性的研究后文献量逐年增长;发表在41种期刊上,以《针灸临床杂志》所刊文献最多;成都中医药大学、辽宁中医药大学对此研究较多;52篇文献受基金项目支持;文献整体被引用量较低,有36篇文章未被引用,最高被引文献被引用频次为105次;17篇临床研究中有10篇随机对照研究;实验研究35篇,主要以各类疾病模型的大鼠为载体,其中消化系统疾病占主体,仅3篇研究健康鼠;临床研究17篇,均以患病人群为载体,以代谢性疾病为主;干预措施以艾灸、电针较多。结论 目前国内有关肠道菌群与针灸治疗效应及作用机理的相关性领域的研究较少,其中多项针灸治疗菌群失调性疾病的机制研究都推测肠道菌群是针灸治疗作用的生理基础之一,针灸可以通过使菌群恢复再平衡而治疗疾病;在临床研究及实验研究中,偏向于观察肠道菌群数量及多样性的变化,较少分析肠道菌群与针灸治疗效应及作用机理之间的关系;此外,临床研究样本量少、半随机或非随机,综述类文献引用目录局限等导致文献质量普遍较低。因此,有必要进一步研究肠道菌群与疾病发生发展的关系,针灸治疗疾病与肠道菌群调控的相关机制及影响因素,以进一步明晰肠道菌群在针灸治病机理中扮演的角色。

Abstract:Objective To explore the situation of current research on the relationship between intestinal flora and the therapeutic effect and mechanism of acupuncture and moxibustion in China, so as to provide references for further clinical and scientific research.Methods Literature from July 1, 1999 to July 1, 2019 was retrieved from CNKI, VIP, WF and PubMed, and extracted according to the predetermined standards to establish a database. Then the quantitative analysis was conducted by using the bibliometrics method.Results A total of 82 pieces of literature were included. Over the years, the number of articles generally showed an overall upward trend after the first literature was published in 2009. All the literature was published in 41 journals, in which the Journal of Clinical Acupuncture and Moxibustion has the most publications. Chengdu University of Traditional Chinese Medicine and Liaoning University of Traditional Chinese Medicine have studied more about this than other universities; 52 pieces of literature were supported by funds. The citation of literature was rare, 36 were not cited, and the highest citation frequency of cited literature is 105 times. Including 10 randomized controlled trials (RCTs) among 17 clinical trials and 33 experimental trials that were mainly based on the various disease model of the rats, of which digestive diseases accounted for the dominant position. Only 3 of the literature took healthy rats as research objects; All of 17 clinical trials took the patients as the subjects, these patients mainly with metabolic diseases; Moxibustion and electroacupuncture were 2 major interventions.Conclusion At present, there are few researches in the field of the relationship between intestinal flora and the therapeutic effect and mechanism of acupuncture and moxibustion in China, among which most researches on the mechanism of acupuncture-moxibustion therapy in the treatment of dysbacteriosis have speculated that intestinal flora is one of the physiological bases of acupuncture-moxibustion therapy, acupuncture can treat diseases by rebalancing the flora. In clinical and experimental researches, they are biased to observe changes in the number and diversity of intestinal flora, and rarely analyze the relationship between intestinal flora and the therapeutic effect and mechanism of acupuncture and moxibustion. In addition, the quality of literature is generally low due to the small sample size, semi-random or non-random of clinical researches, and the limited bibliography of reviewed literature. Therefore, it is necessary to carry out further research to explore the role of intestinal flora in the mechanism of acupuncture-moxibustion treatment by clarifying the relationship between intestinal flora and the occurrence and development of disease and the correlation between the intestinal flora and the pathogenic factors of Chinese medicine.
