个性化钛模板促进兔牙槽骨缺损区即刻牙种植体周围垂直骨量的扩增

摘    要:  目的 探讨个性化钛模板在兔上颌牙槽骨缺损区即刻牙种植后对其周围垂直向骨增量的作用。方法 将8只雄性日本大耳白兔16侧上颌牙槽骨随机分为实验组(8侧)和对照组(8侧)。全麻下拔除2个上颌前磨牙,制作5 mm×10 mm牙槽骨缺损,植入直径2 mm,长8 mm的螺纹钛种植体3 mm,周围植入nHAC,实验组覆盖个性化钛模板,微螺钉固定,对照组不覆盖。术后4周及8周时观察2组种植体周围新骨高度扩增情况,了解X线照片及硬组织切片亚甲基蓝-碱性品红法染色牙种植体骨结合状况。结果 术后4周和8周时,大体标本、硬组织切片亚甲基蓝-碱性品红法染色及X线片均显示种植体骨结合界面良好,但覆盖个性化钛模板组种植体周围牙槽骨高度增加较对照组多。 结论 个性化钛模板具有屏障功能和支撑作用,对种植体周围缺损区可提供一定的时间和空间保障,有利于兔上颌牙槽骨高度的扩增。

Abstract:Objective To investigate the role of individualized titanium template in vertical augmentation of new bone in alveolar bone defect around immediate implant.Methods Sixteen maxillary alveolar bones from 8 male Japanese white rabbits were randomly divided into experimental group(n =8) and control group(n =8).Under general anesthesia,2 maxillary premolars were extracted,and a 5 mm high,10 mm long defect area was established.A screw titanium implant of 2 mm in diameter and 8 mm in length was placed,and was buried into the bone in a depth of 3 mm,and nHAC was put into the defect area around implants.The experimental group was covered with the individualized titanium template,and the control group was placed nothing.At 4 and 8 weeks postoperatively,samples were obtained to observe the vertical augmentation of new bone around implants,X ray films were taken and hard tissue sections were stained to examine the interface between implants and the surrounding new bone.Results At 4 and 8 weeks postoperatively,well osseointegration was found in the interface of implants,and vertical alveolar bone augmentation in experimental group was higher than in control group.Conclusion Individualized titanium template plays an important role of shielding and as a scaffold to provide certain time and space to promote augmentation of alveolar ridge.
