无痛局麻仪在复杂牙拔除术中的应用

目的 探讨无痛局麻仪行局部浸润麻醉拔除复杂牙的麻醉效果。方法 将2015年7～11月合肥市口腔医院收治的43例患者的46颗牙齿随机分为试验组和对照组，每组23颗。试验组应用无痛局麻仪，对照组采用传统卡局式注射麻醉。注射后，患者根据疼痛视觉模拟评分表（VAS）对注射时疼痛进行评价，手术医师结合观察法对麻醉效果进行评价、并记录麻醉和术中的血压及心率，收集数据进行统计分析。结果 对照组VAS评分高于试验组，差异有统计学意义（t=3.01，P<0.05）。试验组麻醉完全率（Ⅰ级）高于对照组，差异有统计学意义（χ²=10.65，P<0.05）。试验组收缩压较对照组低，差异有统计学意义（t=3.30，P<0.05）。两组患者舒张压和心率差异无统计学意义（P>0.05）。结论 无痛局麻仪能明显减轻复杂牙拔除术中患者的疼痛程度和血压波动，提高麻醉效果。

The application of painless oral local anesthetic instrument in complex tooth extraction

Objective To investigate the anesthetic effect of painless oral local anesthetic instrument in pulling out residual roots,residual crowns and fractured tooth. Methods Forty three patients who have to extract tooth were randomly divided into two groups,the experimental group (23 teeth) received painless anesthesia and the control group (23 teeth) underwent traditional anesthesia. The value of visual analogue scale (VAS) and the anesthetic effect were recorded and evaluated. The blood pressure and heart rate of patients were measured and analyzed. Results The higher VAS value and systolic pressure were observed in control group,the anesthetic effect of experimental group was better than that of control group,the difference of diastolic pressure and heart rate in the two groups was not statistically significant. Conclusion The usage of the painless oral local anesthetic instrument could reduce pain and blood pressure,and improve the anesthetic effect in complex tooth extraction.
