长托宁注射液治疗急性有机磷农药中毒的疗效观察

摘    要:目的观察长托宁注射液治疗急性有机磷农药中毒的疗效和安全性,为临床抢救急性有机磷农药中毒患者提供参考。方法将贵州省清镇市人民医院2010年1月至2011年12月收治的240例急性有机磷农药中毒患者完全随机分为试验组(100例)和对照组(140例),试验组应用长托宁注射液治疗(按轻、中、重分别予1~2、2~4、4~6mg肌内注射),对照组采用阿托品注射液治疗(按轻、中、重分别予1—2、3—5、5—10mg静脉注射),观察2组患者的治疗效果及不良反应发生情况。结果试验组治愈率为93.0%(93/100),明显高于对照组的82.8%(116/140)(P〈0.05)。试验组和对照组患者的用药次数、起效时间、症状消失时间、血清胆碱酯酶恢复时间、平均住院时间及不良反应发生例次分别为(7±3)次和(15±5)次、(21±4)rain和(11±2)min、(112±14)h和(164±17)h、(87±8)h和(114±13)h、(8±2)d和(13±3)d、41例次和207例次,2组差异均有统计学意义(均P〈0.05)。结论长托宁注射液治疗急性有机磷农药中毒具有治愈率高、症状消失快、不良反应少等优点,疗效优于阿托品注射液。

Abstract:Objective To observe the effect of penehyclidine hydrochloride injection in the treatment of acute poisoning of organic phosphorus pesticide to provide reference for the clinical treatment of acute organophosphorus pesticide poisoning patients. Methods From January 2010 to December 2011, 240 cases of acute organic phosphorus pesticide poisoning were randomly divided into experimental group (100 cases) and control group (140 cases). Experimental group and control group respectively were treated with penehyclidine hydrochloride and atropine injection, therapeutic effect and the occurrence of adverse reactions were observed. Results In experimental group, the cure rate was 93.0% (93/100), significantly higher than that of control group 82.8% (116/140)] (P 〈0. 05); In 2 groups, the drug times, onset time, the symptoms disappeared time, serum cholinesterase recovery time, average hospitalization time and adverse reaction rates were (7 ± 3 )times and (15 ±5)times, (21 ±4)min and (11±2)min, (112 ±14)h and (164 ±17)h, (87 ±8)h and (114±13)h, (8 ±2)d and ( 13 ± 3 ) d, 41 cases and 207 cases ; the differences were statistically significant ( all P 〈 0.05 ). Conclusion Penehyclidine hydrochloride injection in treatment of acute organic phosphorus pesticide poisoning has a high cure rate which is superior to atropine injection and worthy of clinical application.
