保留半肝动脉血供肝血流阻断技术的应用

目的:探讨减轻肝切除术后肝功能损害的方法.方法:回顾性总结常温下保留半肝动脉血供肝血流阻断下行肝切除术32例经验,并与同期常温下全肝入肝血流阻断行肝切除术38例作对比研究.结果:保留半肝动脉血供肝血流阻断切肝组肝门阻断时间明显长于全肝入肝血流阻断组(P＜0.01),两组术中失血量无差异(P＞0.05).术后3 d肝功能ALT指标,保留半肝动脉血供肝血流阻断切肝组明显低于全肝入肝血流阻断组(P＜0.01).结论:常温下保留半肝动脉血供肝血流阻断行肝切除术是一种可行的方法,对防治术后肝衰竭有重要意义.

Application of Selective Hemihopatic Vascular Occlusion

Objective To explore the methods of reducing liver demage after hepatectomy Methods Restrospectively 32 cases of normothermic selective hemihepatic vasculur occlusion were analyzed as compared with 38 cases of normothermic total hepatic vascular occlusion for hepatectomy Results The time of the selective hemihepatic vascular occlusion was longer than the hepatic vascular occlusion for resection (P<0.01).The amounts of blood loss were not different between the two groups(P>0.05).On the third postoperative day,the alanine aminotransferase(ALT) levels in selective hemihepatic vascular occlusion were lower than those in the hepatic vascular occlusion for resection (P<0.01) Conclusion The method of selective hemihepatic vascular occlusion in mormothermic is feasible and significant in the prevention and treatment of liver failure after operation.
