超声心动图对胎儿主动脉弓峡部血流的频谱分析

目的探讨超声心动图对胎儿主动脉弓峡部血流的频谱分析价值。方法选取我院孕妇156例,行超声心动图检查主动脉峡部内径、血流速度、搏动指数、阻力指数及胎儿左心室排血量、右心室排血量、动脉导管流量以及主动峡部流量情况,并分析上述检测参数与孕周的相关性。结果孕周与峡部内径、收缩早起流速、收缩末期流速、舒张早期流速、舒张中期流速、舒张末期流速、PI、RI呈正相关关系。孕周与胎儿左心室排血量、右心室排血量、联合心输出量、主动脉峡部血流、动脉导管流量呈正相关关系。回归方程结果显示:孕周与峡部内径、收缩早起流速、收缩末期流速、舒张早期流速、舒张中期流速、舒张末期流速、PI、RI拟合程度相对较差,与胎儿左心室排血量、右心室排血量、联合心输出量、主动脉峡部血流、动脉导管流量拟合程度较好。结论正常胎儿超声心动图检查主动脉血流频谱各参数与孕周呈现正相关,其能够为诊断胎儿血流异常提供帮助。

Spectrum analysis of fetal aortic isthmus flow using fetal echocardiography

Objective Fetal echocardiography was used to analyze the frequency spectrum of isthmus of fetal aortic arch,and to explore the situation and characteristics of isthmus of aortic arch in different gestational weeks.Methods 156 pregnant women who were examined by fetal echocardiography from February 2018 to January 2019 in our hospital were selected as the study objects.The pregnant women were 20~40 weeks old and the follow-up results showed that the fetus was normal without diseases and deformities.The diameter of isthmus,blood flow velocity,pulsatility index,resistance index,fetal left ventricular output,right ventricular output,arterial catheter flow and active isthmus flow were analyzed,and the correlation between the above parameters and gestational age was analyzed.Results There was a positive correlation between gestational age and isthmus diameter,systolic velocity,end systolic velocity,early diastolic velocity,middle diastolic velocity,end diastolic velocity,PI,RI.There was a positive correlation between gestational age and fetal left ventricular output,right ventricular output,combined cardiac output,aortic isthmus blood flow and arterial catheter flow.The results of regression equation showed that the fitting degree of gestational week and isthmus diameter,early systolic velocity,late systolic velocity,early diastolic velocity,middle diastolic velocity,late diastolic velocity,PI,RI was relatively poor,and the fitting degree of fetal left ventricular discharge,right ventricular discharge,combined cardiac output,aortic isthmus blood flow and arterial catheter flow was relatively good.Conclusion Normal fetal echocardiography shows a positive correlation between the parameters of aortic blood flow spectrum and gestational weeks,which can help diagnose fetal blood flow abnormalities.In addition,echocardiography can improve the clinical understanding of fetal left ventricular circulatory system,and can be used as a related detection method to identify abnormal pathology.
