乳腺癌bcl-2表达与患者预后相关因素关系的研究

目的：研究乳腺癌中bcl-2蛋白表达与患者生存期及其组织学分级、雌激素受体（ER）、p53、c-erbB-2等影响预后因素的分析。方法：采用免疫组化LSAB法检测bcl-2、ER、p53及c-erbB-2的表达，并分析其相关性及其与预后的关系。结果：bcl-2阳性表达率为50．27％（92／183），其表达与乳腺癌组织学分级、ER状态呈明显正相关，与p53及c-erbB-2表达呈明显负相关；bcl-2阴性者其总生存率及无瘤生存率均明显低于bcl-2阳性者。结论：bcl-2蛋白阴性的乳腺癌患者预后不好，这可能与bcl-2失表达肿瘤中，p53蛋白聚积、c-erbB-2过度表达以及ER的缺失有关；免疫组化检测bcl-2表达可为预测乳腺癌患者预后及选择内分泌治疗提供一个有实用价值的指标。

The Relationship between the Expression of bcl-2 and the Survival Time and Prognostic Related Factors in Breast Cancer Patients

Objective:To study the influence of bcl-2on the survival of patients with breast cancer and the relationship between the expression of bcl-2protein and the histological grade,status of ER,expression of p53,c-erbB-2protein and the prognosis of patients with breast cancer.Methods:By using LSAB immunohistochemistry to detect the expression of bcl-2,ER,p53m,c-erbB-2and to evaluate their correlation with the prognosis.Results:The overall posi-tive rate of bcl-2was50.27%(92/183)and the expression of bcl-2was positively related to the histological grade and status of ER and negatively related to the expression of p53and c-erbB-2protein.The overall survival time and the disease-free survival time of patients without expres-sion of bcl-2were significantly shorter than those with positive bcl-2.Conclusion:In breast can-cer,the patients with negative bcl-2might have a poor prognosis.The bcl-2immunostaining ac-tually proves to be a valuable marker for predicting prognosis and selecting the endocrine therapy.
