女性原发不孕影响因素的病例对照研究

目的探讨已婚育龄妇女原发不孕的影响因素。方法对168例已婚原发不孕患者和孕产妇以1：1配对，就原发不孕的影响因素进行面对面问卷调查，应用SPSS11．5软件进行单因素和多因素条件Logistic回归分析。结果盆腔感染性疾病史（OR=9．475）、药物治疗史（OR=25．644）、促排卵用药史（OR=15．538）、痛经（OR=1．894）及精神压力（OR=2．492）可能是女性原发不孕的危险因素，避孕（主要为避孕套）（OR=0．086）和家庭年人均收入（OR=0．442）可能为其保护性因素。结论预防和治疗盆腔感染，严格掌握促排卵药的适应证，重视生殖健康和避孕知识的宣传教育，增加家庭年人均收入，缓解精神压力，提倡晚婚晚育，避免婚前性生活可以减少女性原发不孕的发生。

A Case-control Study on the Risk Factors of Female Primary Infertility

Objective To explore the risk factors of female infertility in child-bearing aged women. Methods A hospital-based case-control study was conducted with 168 female Infertility cases. One matched controls per case were randomly selected. Both cases and controls were face to face interviewed with a uniformed questionnaire. Conditional logistic regression model was used for univariate and multivariate analysis on SPSS 11.5 to assess odds ratios （OR） and 95 percent confidence internals （95 %CI）. Results Multiple conditional logistic regression analysis of the data showed that the risk factors of infertility were as follows： pelvic inflammatory diseases（OR= 9. 475） ,drug treatment history （OR=25. 644） ,ovulation medication history（OR = 15. 538） ,dysmenorrhea（OR= 1. 894）and mental stress（OR = 2. 492; the protective factors of infertility were contraception（OR=0. 086）and average annual income of families（OR=0. 442）. Conclusion Prevention and treatment of pelvic infection, application of strictly controlled ovulation drugs, popu larization of sexual and reproductive healthy educations and relief of mental stress would be im portant measures of declining the incidence of infertility.
