77例成人不典型肺结核的临床分析

目的探讨成人不典型肺结核的临床体征、肺部特征、影像学检查特征、实验室检查情况。方法选取本院2010年12月至2011年12月收治的成人不典型肺结核患者77例，对这些患者的临床诊断、检查资料进行回顾性总结分析。结果本组77例患者临床症状表现为咳嗽40例（51．95％），发烧23例（29．87％），胸闷32例（41．56％），咯血12例（15．58％），胸部疼痛23例（29．87％），四肢乏力22例（28．57％），胸部积水29例（37．66％），没有明显症状28例（36．36％）；本组患者临床肺部体征表现为呼吸音减弱10例（12．99％），湿罗音17例（22．08％），其中没有肺部体征50例（64．94％）；影像学检查表现存在叶或段状分布的实质性病变或者肺不张特征38例（占49．35％），结节或者肿块样病变20例（占25．97％），不典型空洞11例（占14．28％），弥漫性粟粒结节8例（占10．39％）；实验室检查表现白细胞升高12例（15．58％），白细胞正常38例（48．05％），白细胞减少27例（35．06％），血沉增快56例（72．73％），结核菌素试验强阳性12例（15．58％），血结核抗体阳性46例（59．74％）。结论对于不典型肺结核病例的临床诊断应进行相关检查并结合患者的临床体征进行综合判断。

77 cases of adult atypical pulmonary tuberculosis cfinical analysis

Objective To investigate the clinical signs of adult atypical pulmonary tuberculosis, fea- tures, image examination, laboratory examination characteristics. Methods In our hospital, from December 2010 to December 2011, 77 cases of adult atypical pulmonary tuberculosis patients, were retrospectively ana- lyzed the data of clinical diagnosis, examination of these patients. Results In this group, 77 cases of patients with clinical symptoms such as cough, have a fever in 40 cases （51.95%） of 23 cases （29. 87% ）, 32 cases of chest congestion （41.56%）, 12 cases of haematemesis （ 15.58% ）, chest pain in 23 cases （29. 87% ） , 22 cases of weak limbs（28.57% ）, 29 cases of thoracic hydrops （37. 66% ）, no obvious symptoms of 28 cases （36. 36% ） ; the groups of patients with clinical physical signs of the lungs showed decreased breath sounds in 10 cases （12. 99% ）, 17 cases of moist rales （22. 08% ）, 50 cases of no pulmonary symptoms （64. 94% ）. Im- aging performance in the presence of lobar or segmental distribution of the essence lesions or atelectasis features of 38 cases （49. 35% ）, nodular or mass lesions in 20 cases （25.97%）, atypical cavity 11 cases （ 14. 28% ）, diffuse miliary nodules in 8 cases （10. 39% ） ,laboratory examination showed the increase of white blood cell in 12 cases （ 15.58% ）, normal white blood cells 38 cases （48.05%）, 27 cases of leukopenia （35.06%）, e- rythrocyte sedimentation rate increased in 56 cases （72.73%）, strongly tuberculin test was positive in 12 cases （ 15.58% ）, serum tuberculosis antibody was positive in 46 cases （59. 74% ）. Conclusion for the clinical di- agnosis of atypical pulmonary tuberculosis cases should be related with clinical signs in patients with inspection and comprehensive judgment.
