老年患者床边螺旋推进法盲插鼻肠管

目的探讨安全、简便的适合老年患者床边螺旋形鼻肠管盲插的方法。方法将56例老年患者随机分为对照组和观察组各28例。对照组采用传统方法插管,等待鼻肠管自动移行至十二指肠或空肠上段;观察组置管前先滴注红霉素,15min后采用螺旋推进法行鼻肠管置管,确认鼻肠管在胃内后,将空气按5～10ml/kg注入胃内,再螺旋推进置管50cm,每次进管2～4cm,旋转45°。结果观察组置管成功率显著高于对照组,置管深度显著长于对照组,自置管起至予肠内营养的等待时间显著短于对照组(均P0.01)。结论螺旋推进置管法行床边盲插鼻肠管操作简单,置管成功率高,操作耗时短、安全。

Bedside blind placement of nasojejunal tubes by using spiral placement with insufflation air technique in elderly patients
