CT灌注成像指导下动脉内溶栓治疗急性缺血性脑卒中的研究

目的：探讨在CT灌注指导下动脉内溶栓治疗急性缺血性脑卒中的可行性,并对其疗效进行评估。方法：对44例急性缺血性脑卒中患者行头颅CT平扫及CT灌注成像检查,并计算各灌注参数值。对存在缺血半暗带患者给予动脉溶栓治疗,并分别于溶栓前、溶栓后即刻、溶栓后3周和3个月进行NIHSS评分,以评估近期及远期疗效。结果：入组患者经动脉内溶栓治疗取得较好疗效,随访3个月患者恢复良好,NIHSS评分有明显改善。结论：CT灌注成像能够快速、准确反映缺血部位及范围,为指导急性缺血性脑卒中患者进行及时有效的动脉内溶栓治疗以及疗效和愈后的评价提供了可靠依据。

CT perfusion guided arterial thrombolysis for acute ischemic stroke patients

Objective： To evaluate the guiding effect of CT perfusion in arterial thrombolytic therapy,and assess its feasibility and efficacy.Methods： Non-contrast CT scan and CT perfusion imaging were performed on 44 patients with acute ischemic stroke,and perfusion parameters were calculated then.Where ischemic penumbra existed,the intra-arterial thrombolysis was administered.It was assessed by NIHSS score before and after thrombolytic therapy at the 3rd week and the 3rd month.Results： Patients treated by the intra-arterial thrombolysis gained better efficacy,3-month following-up showed better recovery,and NIHSS score was significantly improved.Conclusion： CT perfusion imaging can rapidly and accurately reflect the location and extent of ischemia,and intra-arterial thrombolytic therapy is safe and effective.
