从MRI的表现探讨鼻咽癌生长方式及淋巴结转移的规律

目的　探讨鼻咽癌 (NPC)的生长方式及淋巴结转移的规律。方法　搜集经病理活检证实且检查前未做任何治疗的 83例NPC ,通过MRI影像表现分析NPC的生长模式及淋巴结转移与NPC深层结构侵犯的关系。结果　NPC的 3种生长方式与淋巴结转移的关系无显著性意义 ;颈部淋巴结转移有两侧转移的趋势 ,咽后淋巴结有单侧转移的趋势 ;茎突后间隙受累有较高的颈部淋巴结转移率。结论　MRI可以确切了解NPC的生长方式、侵犯范围、有无淋巴结转移 ,对NPC的分期及放疗计划的制定有重要作用。

MRI study of the growth's way and the rule of lymph node metastasis of nasophgeal carcinoma

Objective To study the growth’s way and the rule of lymph node metastasis of nasopharyngeal carcinoma(NPC).Methods There were 83 cases of naspharyngeal carcinoma,the diagnosis were confirmed by pathology ,and MRI scanning was performed in all patients before radiotherapy.The growth way of NPC and the relationship between lymph node metastasis and the deep structure’s involovement were analyzed by MRI.Result The growth’s way of NPC therapy schedule markedly relation to lymph node metastsis.The cervical lymph metastsis was easely to two sides,retropharyngeal lymph node metastsis showned the trends of single side metastasis;the involvement of posterior space of styloid process was connection with cervical lymph metastsis.Conclusion MRI may know definitively the growth’s way,involvement structure and lymph metastasis,and it is important in the grading stage and instituting radio therapy plan for NPC patients.
