HASTE法磁共振胰胆管造影诊断胆道病变的应用价值

目的：探讨HASTE法单次屏气磁共振胰胆管造影（HASTE－MRCP）诊断胆道病变的应用价值。方法：对照分析75例胆道病变患者的HASTE－MRCP影像和手术结果，并与经皮肝穿刺胆管造影（PTC）比较。结果：HASTE－MRCP诊断胆道病变，正确率96％（72／75）；诊断胆管恶性梗阻灵敏度97．1％，特异率95％，阳性预测值94．4％，阴性预测值97．4％。35例恶性胆管梗阻中，HASTE－MRCP显示癌肿32例，其癌肿发现率高于PTC（8／35）。结论：HASTE－MRCP在胆道病变诊断中具有准确的定性诊断价值。

The application value of HASTE sequence magnetic resonance cholangiopancreatography in diagnosing biliary tract diseases

Objective To evaluate the clinical application of half-Fourier acquisition single - shot turbo SE-sequence single breah - hold magnetic resonance cholangiopancreatography (HASTE - MRCP) in diagnosing biliary tract diseases. Methods HASTE - MRCP was performed in 75 patients with biliary tract diseae. HASTE - MRCP findings was compared with those of percutaneous transhepatic cholangiography (PTC) and surgical outcome. Results Diagnostic HASTE- MRCP examinations were performed in all 75 patients. HASTE - MRCP yielded an accuracy of 96% in diagnosing biliary tract disease (n=72).The sensitivity, specificity, positive predictive value and negative predictive value of HASTE-MRCP in diagnosing malignant obstruction of bile duct was 97. 1%, 95%, 94.4% and 97.4%, respectively. In 35 cases of malignant obstruction, 32 cases were depicted the carcinomatic mass in HASTE- MRCP, while 8 were delineated the mass in PTC. The detection rate of the mass of cholangiocarcinoma by HASTE - MRCP was higher than that by PTC(P < 0.01). Conclusion HASTE-MRCP enables to accurately evaluate biliary tract disease and make definite diagnoses, which is a potential alternative of PTC in a large extent.
