腹式筋膜内全子宫切除102例临床分析

摘    要:目的: 观察腹式筋膜内全子宫切除术治疗子宫良性病变患者的临床疗效,寻找一种更安全、简捷的术式。方法: 102例子宫良性病变患者(实验组),采取筋膜内电刀旋切全子宫术,100 例(对照组),采取筋膜外全子宫切除术。记录手术时间、术中出血量、术后排气时间、术后住院天数及随访情况。结果: 实验组的术中出血量明显少于对照组(P<0.01),术后排气时间及住院天数明显短于对照组(P<0.01)。实验组术中直肠陷凹病变率明显高于对照组(P<0.005)。结论: 筋膜内电刀旋切全子宫术优于传统术式,对有子宫切除指征而无恶性倾向的患者,尤其是直肠陷凹有病变及有手术史的患者,筋膜内电刀旋切全子宫术可作为一种更安全、更简捷的全子宫切除术术式。

Abstract:Objective: To discuss about one of the most safe, simple types of total hysterectomy, intrafascial total hysterectomy, and its clinical advantages to treat the benign lesions in the uterus. Methods: 102 patients with benign lesions In the uterus were operated by intrafacial total hysterectomy. While another 100 patients in the control group were operated by extrafascial total hysterectomy; operating time, amounts of bleeding during operation, recovering time of the gastrointestinal mobility, and the number of days in hospital were recorded The conditions of patients after operation were also evaluated. Results: The amounts of bleeding, recovering time of the gastrointestinal mobility, and the number of days in hospital in Intrafascial total hysterectomy group which was performed by electric knife were significantly lower than those in control group. Conclusion: Intrafascial abdominal total hysterectomy is better than traditional total hysterectomy with its several advantages such as safe, simple suitable to treatment.
