老年脑卒中患者营养状况的评定与影响因素分析

摘    要:目的:评定老年脑卒中患者的营养状况,分析与老年脑卒中患者营养风险、营养不良发生相关的因素。方法:以2012年5月—2013年11月入住中日友好医院老年科、≥65岁、住院时间≥24h的208例脑卒中患者为研究对象。使用微型营养评价精法(MNA-SF)对研究对象进行营养评定,使用统一调查问卷对影响因素进行调查。结果:208例老年脑卒中患者营养风险的检出率为41.3%、营养不良的检出率为26%,随着年龄的增加,营养风险、营养不良的发生率增加,差异有统计学意义(P0.05);营养风险和营养不良的发生与罹患脑卒中时间、日常生活活动能力、家庭照料情况相关(P0.05)。结论:老年脑卒中患者营养状况差,存在营养风险和营养不良的患者患病时间长、生活活动能力低、家庭照料差。关注老年脑卒中患者营养状况,控制影响因素,有利于脑卒中预后。

Abstract:Objective] To evaluate nutritional state of elderly stroke patients and analyze influencing factors about nutritional risk and malnutrition. Method] Totally 208 stroke inpatients aged ≥65years, hospitalization time ≥24h in geriatrics in China-Japan Friendship Hos- pital from May 2012 to November 2013 were consecutively enrolled. Their nutritional states was evaluated by MNA-SF and influencing factors were investigated by questionnaire Result] The results showed that the incidence of nutritional risk and malnutrition were 41.3% , 26% re- spectively, nutritional state was significantly influenced by ages ( P 〈 0. 05 ) , incidence of nutritional risk and malnutrition was associated with duration of stroke, activity of daily living, and family care (P 〈 0. 05 ) . Conclusion] There was high incidence of nutritional risk and malnutrition in elderly stroke patients. Malnourished elderly stroke patients were always accompanied with long duration of stroke, poor activity of daily living, less family care. Paying more attention to nutritional states and controlling influencing factors will improve the progno- sis of elderly stroke patients.
