急性冠状动脉综合征患者阿司匹林抵抗及危险因素研究

目的研究急性冠状动脉综合征(ACS)患者阿司匹林抵抗(AR)的临床特征及其危险因素。方法对165例ACS患者、22例稳定型心绞痛(SAP)患者和16例对照者的AR情况进行比较分析,并测定血液生化指标和记录心血管疾病危险因素、伴随疾病、超声心动图、冠状动脉造影结果及介入治疗等。结果对照组AR发生率为12.5%,SAP组AR发生率为36.4%,ACS组AR发生率为54.5%,ACS组与SAP组和对照组AR发生率间差异均有统计学意义(P<0.05);AR组和阿司匹林敏感(AS)组患者的高敏C-反应蛋白(hs-CRP)、总胆固醇、低密度脂蛋白-胆固醇水平和病变血管数、严重病变数及糖尿病率、介入治疗率间差异均有统计学意义(P<0.05);AR与合并糖尿病、hs-CRP水平升高和低密度脂蛋白-胆固醇升高相关。结论ACS患者AR发生率高于冠状动脉造影正常者及SAP患者,合并糖尿病、hs-CRP和LDL水平升高是AR的危险因素。

Aspirin Resistance and Risk Factors in Acute Coronary Syndrome Patients

Objective To study the clinical feature of aspirin resistance(AR)in patients with acute coronary syndrome(ACS)and the correlated risk factors of AR.Methods The AR conditions of 165 ACS patients,22 patients with stable angina pectoris(SAP,)and 16 normal controls were comparatively analyzed.The blood biochemical indicators were measured,and the risk factors,concomitant diseases,ultrasonic echocardiogram,the results of coronary arteriongraphy and interventional therapy of the cardiovascular diseases were also recorded.Results The AR morbidity was 12.5% in the control group,36.4% in the SAP group,and 54.5% in the ACS group,with significant differences between the control group,SAP group and ACS group(P<0.05).Between the AR patients and aspirin sensitive patients,there were significant differences in the levels of hsCRP,CHO and LDL-C;in the numbers of diseased coronary artery and serious lesions;in the rate diabetes attack;and in the rate of interventional therapy(P<0.05).AR in ACS was correlated with diabetes complication,increase of hs-CRP level and increase of LDL-C.Conclusion ACS patients had more increased AR than SAP patients and persons with normal coronary arteriongraph.Diabetes complication,high levels of hs-CRP and LDL-C were risk factors of AR.
