Q-开关激光联合强脉冲光治疗黄褐斑临床疗效分析

目的探讨和评价Q-开关激光联合强脉冲光（IPL）治疗黄褐斑的临床疗效和安全性。方法激光治疗组35例，采用MedliteQ-开关激光治疗6次，间隔1个月治疗一次；激光与强脉冲光联合治疗组42例，先用MedliteQ-开关激光治疗6次，间隔1个月治疗一次，再用Lumenisone强脉冲光治疗5次，间隔1个月治疗一次；对照组12例，采用中药祛斑面膜治疗，每周1次，连续4周一个疗程，总共完成6个疗程。所有患者严格防晒，勿过度劳累。结果黄褐斑对照组治愈0例，显效3例（25．00％），总有效率为25．00％；激光治疗组治愈6例（17.14％），显效16例（45．71％），总有效率为62．85％；激光与强脉冲光联合治疗组治愈10例（23．81％），显效23例（54．76％），总有效率为78．57％。激光治疗组和联合治疗组都取得了显著的疗效（P〈0．05）。对照组红斑过敏1例（8．33％）；激光治疗组色素沉着2例（5．71％）；联合治疗组色素沉着4例（9．52％），色素减退2例（4．76％），未见增生性瘢痕。结论Q-开关激光和强脉冲光（IPL）治疗黄褐斑安全、有效，副作用少，联合治疗能提高疗效。

Clinical effect of laser and intense pulsed light in the treatment of melasmas

Objective To explore and evaluate the curative effect and safty of laser and intense pulsed light in the treatment of melasmas. Methods 89 patients with melasma were treated. 35 melasmas were initially treated with Q - switched laser with 1064nm wavelength, followed by six times consecutive treatments, once a month. Other 42 melasmas firstly received 6 times consecutive treatments of Q - switched laser with 1064nm wavelength, then were treated by 5 times consecutive treatments with IPL once a month. And still other twelve melasmas were consistent of the control team received 24 times consecutive treatments of traditional Chinese medicine facial mask, once a week. All of patients were strict with sun - proof. Results The curative effect of the control team, the treatment team of laser and the treatment team of laser and IPL were 25%, 62.85% and 78.57%, respectively. The side - effects were also observed. Erythema was 8.33% in the control team. Hyperpigmentation was 5.71% and 9.52% in the treatment team of Laser and the treatment team of Laser and IPL. Hypopigmentation is 4.76% in the treatment team of Laser and IPL. Hypertrophic scar was not found. Conclusion Laser and IPLwere effective and safe in treatment of melasmas.
