GPC3在原发性肝细胞癌中的表达及其临床病理相关性研究

摘    要:目的 测定磷脂酰肌醇蛋白聚糖3(Glypican-3,GPC3)在原发性肝细胞癌、癌旁组织、肝硬化组织及正常肝组织中的表达,分析其与原发性肝癌临床病理特征的关系.方法 筛选赣州市人民医院肝胆外科近5年手术切除并经病理确诊为肝细胞癌患者43例(术前均未经任何抗肿瘤治疗),应用免疫组化通用型二步法检测并对比GPC3在原发性肝细胞癌组织、癌旁组织、肝硬化组织及正常肝组织中的表达情况.同时对比分析GPC3和AFP在原发性肝细胞癌中的阳性表达率.结果 1.GPC3在肝细胞癌组织中阳性表达率高,与在癌旁组织、肝硬化组织及正常肝脏组织中表达差异有统计学意义(P<0.05).2.GPC3表达与肿瘤分化程度有关,差别有统计学意义(P<0.05).3.GPC3与AFP在肝细胞癌组织中的表达比较,GPC3对肝癌免疫组化诊断敏感性更高(P<0.0005).结论 1.与目前应用最广泛的肿瘤标志物AFP相比,GPC3具有更高的免疫组化诊断敏感性,有望成为一种新的免疫标志物应用于肝癌的临床免疫组化诊断和鉴别诊断.2.GPC3表达强度与肝癌患者肿瘤细胞分化有一定相关性.

Research of the Glypican-3 Expression in Hepatocelluar Carcinoma and its Clinicopathogical Relevance

Abstract:Objective To observe the expressions of Glypican-3 in hepatocellular carcinoma, pa- raneoplastic, cirrhosis and normal tissue so as to observe its relationship to clinicopathogical characteristics in hepatocellular carcinoma (HCC), and to provide the prognosis evaluation in HCC.Methods Immunohistochemistry was used on the screened 43 cases of patients with hep- atocellular carcinoma to detect the expression of Glypican-3 in hepatocellular carcinoma, cir- rhosis and normal controls, carcinoma tissues according to their clinical and pathological data while analyzing the GPC3 and AFP positive expression rate in hepatocellular carcinoma (HCC). Results 1.Expression of GPC3 in tumor was significantly higher than those in paraneoplastic, cir-rhosis and normal tissue. (P〈0.05)2.GPC3 expressions in carcinoma tissues were correlated with differentiation (P〈0.05). 3.Compared with AFP,GPC3 was more sensitive for the immu- nohistochemistry diagnosis of HCC(P〈0.05).Conclusion 1.Compared with the wildly used immune marker AFP,GPC3 is more sensitive and could be used as a new immune marker for the immunohistochemistry diagnosis and differential diagnosis of HCC. 2.GPC3 may be related to the differentiation of tumor cells.
