中央脊髓综合征的手术治疗进展

中央脊髓综合征(CCS)是最常见的急性不完全性颈脊髓损伤,其主要临床特点为上肢运动功能障碍远较下肢显著、膀胱功能障碍以及脊髓损伤平面以下不同程度的感觉丧失。CCS好发于并存颈椎退变的老年人,颈椎过伸性损伤是CCS的主要病因。关于CCS的治疗,以往多主张以非手术治疗为主。然而,近年来的研究证实,对CCS患者进行手术治疗是安全和有效的。该文就CCS的发病机制、病理生理及手术治疗等的研究进展予以综述。

Advances in Surgical Treatment of Central Cord Syndrome

Central cord syndrome(CCS)is the most common incomplete cervical spinal cord injury.Patients with CCS present with disproportionate impairment of the upper much severer than in the lower extremities,bladder dysfunction,and varying degrees of sensory loss below the level of the lesion.The classic scenario is an older patient with underlying cervical stenosis and spondylosis who falls and incurs a hyperextension injury.Previously,most surgeons advocated non-surgical treatment for CCS.However,recently numerous studies have demonstrated that surgical treatment in the setting of CCS is safe and efficacious.Here reviews the progress of pathogenesis,pathophysiological research and surgical treatment of traumatic CCS.
