再生蛋白4的相关表达在胰腺癌诊断中的意义

目的：探讨检测血清中再生蛋白4（REG4）水平在胰腺癌诊治中的作用。方法：分别采用ELISA法和化学发光免疫法对84例胰腺癌血清REG4和CA19-9进行测定,并与58例胰腺良性疾病和70名正常人进行比较,分析REG4对胰腺癌的临床诊断价值。结果：胰腺癌病人血清REG4水平（2.34±1.37）μg/L]明显高于胰腺良性疾病（0.82±0.57）μg/L]（P〈0.001）和正常人（0.73±0.43）μg/L]（P〈0.001）;Ⅰ＋Ⅱ期胰腺癌REG4升高的病人数明显高于CA19-9升高的病人数（P=0.031）。联合检测血清REG4和CA19-9可将诊断灵敏度提高至90.5%,术后监测表明,胰腺癌切除术后血清REG4下降。血清REG4表达与胰腺癌细胞REG4表达明显相关（χ2=7.54,P〈0.001）。结论：血清REG4是一个潜在的胰腺癌血清标志物,对胰腺癌的诊断和随访具有临床价值。

Significance of serum REG4 upregulation in the diagnosis and treatment of pancreatic adenocarinoma

Objective To study the serum level of REG4 in the diagnosis and prognosis of pancreatic adenocarcinoma.Methods Serum REG4 and CA19-9 was were examined in 84 patients with pancreatic adenocarcinoma by using ELISA and luminescence immunoassay,respectively.Fifty-eight patients with begnign pancreatic disease and 70 healthy controls were studied at the same time.Results Compared with patients of benign pancreatic disease and healthy controls,the serum level of REG4 was higher in pancreatic cancer patients（P0.001,respectively）.In patients with stages Ⅰ Ⅱ pancreatic adenocarcinoma,their serum REG4 was upregulated more severely than their serum CA19-9（P=0.031）.Concurrent testing of REG4 and CA19-9 could increase the diagnostic sensitivity to 90.5%.REG4 was decreased significantly after radical tumor resection.In surgical specimens,immunohistochemical analysis confirmed the correlation between the serum REG4 level and the REG4 expression in pancreatic cancer tissues.Conclusions Serum REG4 has potentially useful as a serum marker in the diagnosis and in the follow-up of operated cases of pancreatic cancer.
