缺氧缺血性脑损伤患儿康复训导手册的建立与应用

目的探讨缺氧缺血性脑损伤（HIBD）小儿康复训导手册应用于临床的治疗效果。方法选取2001—2005年140例HIBD患儿，半随机分成康复训练组和对照组。待病情稳定后，按自行设计的HIBD0～2岁康复训导手册的训练方法，采取儿科专业医师指导下的医院家长合作治疗HIBD患儿模式进行康复训练治疗2年。用临床、CT资料和发育商（DQ）、智力指数（MI）评价治疗效果。结果通过对照、自身前后对照研究两组的临床、CT资料，动态研究两组DQ、MI值，结果显示康复训练组DQ、MI值明显高于对照组（P〈0．01），91．43％患儿临床恢复正常，71．43％患儿CT恢复正常，康复训练组的预后不良率明显低于对照组（P〈0．01）。结论康复训导手册指导下的康复训练能有效地防止和减轻HIBD患儿的后遗症和减少伤残，从而改善预后。

Establishment and application of rehabilitation training handbook for children with hypoxic ischemic brain damage

Objective To investigate clinical therapeutic efficacy of the rehabilitation training handbook for children with hypoxic ischemic brain damage （HIBD）. Methods 140 children with HIBD during 2001 to 2005 were divided into rehabilitation training group and control semi-randomly. After the stabilization of patient＇s condition,the children were trained for 2 years by co- operation of the doctors and children＇s parents who were directed by pediatrician using self-designed methods for 0 ~ 2 years old of the rehabilitation training handbook. Clinical therapeutic efficacy was evaluated by clinics, computer tomography （ CT）, develop- ment quotient（DQ） and mentality index （MI）. Results The clinics, CT, DQ and MI between the rehabilitation training and con- trol groups had significant deviation. The clinies,CT,DQ and MI of the rehabilitation group were significant higher than those of the control （P〈0. 01 ）. In the rehabilitation group,91.43 % children were clinical recovered and CT of 71.43 % children was re- covered. Unfavorable prognosis rate of rehabilitation group was significant lower than that of control（P〈0. 01 ）. Conclusion Re- habilitation training directed by rehabilitation training handbook can effeetivdy prevent and relieve the sequelae of the children with HIBD and diminish disability,thus improve prognosis.
