新生儿高间接胆红素血症血清NSE含量及脑干听觉诱发电位变化及意义

摘    要:目的]探讨血清神经元特异性烯醇化酶(NSE)及脑干听觉诱发电位(BAEP)在评价新生儿高间接胆红素血症合并脑组织损伤的临床意义。方法]应用ELISA法分别测定72例高间胆新生儿和30例正常足月新生儿血清s-NSE含量,并对所有对象于生后d7进行脑干听觉诱发电位(BAEP)检测。结果]各组新生儿uSB和s-NSE水平比较差异有统计学意义(F=98.32,73.51、P﹤0.01),两两比较除中、重度高间胆组间比较差异无显著性外,余差异有统计学意义(P﹤0.05)。各组BAEP异常发生率比较差异有统计学意义(χ2=73.51,P﹤0.01),两两比较除中度高间胆组与重度高间胆组间差异有统计学意义(P﹤0.05)。实验组中血清高间胆红素水平与血清NSE浓度与相对应BAEP异常发生率均呈显著性正相关。结论]高间接胆红素血症可导致新生儿听神经损伤,NSE与BAEP在高间胆红素血症所致神经毒性评价方面均具有较高敏感性,可作为临床重要监测指标。
