纤维支气管镜对呼吸机相关肺炎重症患者的效果影响

目的:探讨纤维支气管镜对重症呼吸机相关肺炎患者的治疗效果。方法:选取2018年5月-2019年12月我院收治的90例呼吸机相关肺炎重症患者作为样本对象,将其根据信封法分为两组,对照组(n=45)采用常规灌洗吸痰,观察组(n=45)采用纤维支气管镜进行灌吸吸痰治疗,对比两组治疗效果。结果:观察组总有效率95.6%明显高于对照组71.1%(P<0.05);两组临床肺部感染各项目评分经SPSS20.0软件统计显示差异显著(P<0.05);治疗后观察组气道峰值吸气压力(PIP)、气道阻力(Raw)、呼吸做功(WOB)与对照组相比较低,动态肺顺应性(DLC)较高(P<0.05);观察组有创机械通气时间、ICU入住时间短于对照组(P<0.05)。结论:纤维支气管镜治疗呼吸机相关肺炎重症患者的疗效确切,可改善炎性反应、肺功能及感染程度,预后良好。

Effect of fiberoptic bronchoscopy on severe patients with ventilator-associated pneumonia

objective:to investigate the therapeutic effect of fiberoptic bronchoscopy on severe ventilator-associated pneumonia.Methods:From May 2018 to December 2019,90 patients with severe ventilator-associated pneumonia were selected as samples,Divide it into two groups according to the envelope method,Control group(n=45),Observation group(n=45)was treated with bronchofiberscope for sputum aspiration,The therapeutic effect of the two groups was compared.Results:The total effective rate of observation group was 95.6%,which was significantly higher than that of control group(71.1%,P<0.05);SPSS20.0 software showed that there were significant differences in clinical pulmonary infection scores between the two groups(P<0.05).After treatment,the peak airway inspiratory pressure(PIP),airway resistance(RAW)and respiratory work(WOB)in the observation group were lower than those in the control group,while the dynamic lung compliance(DLC)was higher in the observation group(P<0.05).The duration of invasive mechanical ventilation and ICU stay in the observation group were shorter than those in the control group(P<0.05).Conclusion:Fiberoptic bronchoscopy is effective in treating severe cases of ventilator-associated pneumonia,Can improve inflammatory response,lung function and infection,The prognosis is good.
