慢性阻塞性肺疾病急性加重期伴呼吸衰竭患者机械通气治疗失败的影响因素分析

目的探讨慢性阻塞性肺疾病急性加重期(AECOPD)伴呼吸衰竭患者机械通气治疗失败的影响因素。方法回顾性分析2018年6月至2020年6月我院收治的57例行机械通气治疗的AECOPD伴呼吸衰竭患者的临床资料,采用单因素和多元Logistic回归分析探讨机械通气治疗失败的影响因素。结果57例AECOPD伴呼吸衰竭患者中,机械通气治疗失败率为31.58%(18/57)。单因素和多元Logistic回归分析显示,血清NT-proBNP高表达、合并基础疾病、低pH值是AECOPD伴呼吸衰竭患者机械通气治疗失败的独立影响因素(OR>1,P<0.05)。结论AECOPD伴呼吸衰竭患者机械通气治疗失败与血清NT-proBNP高表达、合并基础疾病、低pH值密切相关,临床需对上述危险因素重点关注。

Analysis on the Influencing Factors for Treatment Failure of Mechanical Ventilation in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease Complicated with Respiratory Failure

Objective To explore the influencing factors for treatment failure of mechanical ventilation in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated with respiratory failure.Methods The clinical data of 57 patients with AECOPD complicated with respiratory failure treated by mechanical ventilation in our hospital from June 2018 to June 2020 were retrospectively analyzed.Univariate analysis and multivariate Logistic regression analysis were used to explore the influencing factors for treatment failure of mechanical ventilation.Results Among 57 patients with AECOPD complicated with respiratory failure,the treatment failure rate of mechanical ventilation was 31.58%(18/57).Univariate analysis and multivariate Logistic regression analysis showed that high expression of serum NT-proBNP,complication of underlying diseases and low pH value were independent influencing factors for treatment failure of mechanical ventilation in patients with AECOPD complicated with respiratory failure(OR>1,P<0.05).Conclusions The treatment failure of mechanical ventilation in patients with AECOPD complicated with respiratory failure is closely related to the high expression of serum NT-proBNP,complication of underlying diseases and low pH value.Clinical attention should be paid to the above risk factors.
