灌注溶栓联合血栓抽吸治疗急性动脉栓塞的临床研究

目的：探讨经导管灌注溶栓联合血栓抽吸治疗急性动脉栓塞的临床价值。方法对急性动脉栓塞患者进行经导管灌注溶栓联合血栓抽吸治疗，与前期单纯经导管溶栓治疗的患者行回顾性对比分析，比较两组在显效率、显效时间、截肢率、严重出血率等方面的差异。结果单纯经导管溶栓组（ A组）14例，灌注溶栓联合血栓抽吸（ B组）18例。与A组相比，B组显效率明显提高（88.89% vs 78.57%），显效时间明显缩短（8.4＆#177;6.8）h vs（13.6＆#177;8.5）h]，截肢率明显减少（5.56% vs 14.29%）。另外，A组肌筋膜综合征及严重出血的发生率也明显高于B组。结论经导管灌注溶栓联合血栓抽吸治疗急性动脉栓塞安全、有效，有较好的临床应用价值。

Infusion thrombolysis combined with thrombus aspiration in the treatment of acute arterial embolism

Objective To investigate the clinical value of transcatheter infusion thrombolysis combined with thrombus aspira-tion on acute arterial embolism. Methods Patients with acute arterial embolization were treated by transcatheter infusion throm-bolysis combined with thrombus suction,and were retrospectively analyzed with the previous cases who were only treated by tran-scatheter thrombolysis. The differences in the marked effective rate,effective time,amputation rate and serious bleeding rate of the two groups were compared. Results Fourteen cases in transcatheter thrombolysis group（ group A）,and 18 cases in infusion thrombolysis combined thrombus aspiration group（ group B）. The marked effective rate increased more significantly in group B than that in group A（88. 89% vs 78. 57%,P﹤0. 05）,the effective time was significantly shortened in group B than in group A （8. 4 ＆#177;6. 8）hours vs（13. 6 ＆#177; 8. 5）hours],the amputation rate decreased more significantly in group B than in group A （5. 56% vs 14. 29%）. In addition,the incidences of myofascial syndrome and severe bleeding were obviously higher in group A than those in group B. Conclusion Transcatheter infusion thrombolysis combined thrombus aspiration in the treatment of acute arterial embolization is safe and effective,it has better clinical application value.
