综合护理干预对预防下肢骨折手术术后并发深静脉血栓形成的影响

目的 探讨综合护理干预对预防下肢骨折手术术后并发深静脉血栓形成的影响。方法 选取2013年6月~2014年6月南京市江宁医院收治的86例下肢骨折患者,将其分为对照组和观察组,每组各43例。对照组患者进行常规护理干预,观察组患者在对照组的基础上进行综合护理干预。对两组患者术后深静脉血栓形成发生率、住院时间和下床时间、治疗依从性及护理满意度进行比较。结果 1观察组深静脉血栓形成发生率为4.7%（2例）,其发生率显著低于对照组（9.3%,4例）,差异有统计学意义（χ^2=7.593,P=0.024〈0.05）。2观察组的住院时间及下床时间明显短于对照组（13.7±2.2）h比（2.8±1.0）h,（18.1±2.9）h比（2.1±0.7）h],差异均有统计学意义（均P〈0.05）。3干预后观察组患者的服药依从性、健康教育依从性及出院患者复诊依从性均明显高于对照组,差异均有统计学意义（均P〈0.05）。4观察组的护理满意度（95.3%）明显高于对照组（83.7%）,差异有统计学意义（P〈0.05）。结论 对下肢骨折手术患者实施综合护理干预,可明显减少术后深静脉血栓的形成,缩短住院时间,提高治疗依从性及护理满意度,值得临床推广应用。

Influence of comprehensive nursing intervention on the prevention of lower limb fracture surgery complicated by deep vein thrombosis

Objective To investigate effect of comprehensive nursing intervention on the prevention of deep venous thrombosis after the surgery of lower limb fractures. Methods Totally 86 cases of patients with lower limb fractures in Jiangning Hospital of Nanjing City from June 2013 to June 2014 were divided into observation group and control group,with 43 cases in each group. Control group was given routine care, while observation group was given comprehensive nursing intervention on the basis of the control group. The incidence of deep venous thrombosis, days of hospitalization and down bed time, compliance and satisfaction degree of patients in two groups were compared. Results 1The incidence of deep venous thrombosis in observation group was significantly lower than that in control group, the difference was statistically significant 4.7%（2 cases） vs 9.3%（4 cases）, χ^2=7.593, P = 0.0240.05]. 2Days of hospitalization and getting out of bed in observation group were significantly shorter than those in control group, the differences were statistically significant （13.7±2.2） h vs（2.8±1.0） h,（18.1±2.9） h vs（2.1±0.7） h]（P〈0.05）. 3The compliance of medication adherence and hospital patients, health education compliance appointment in patients in observation group were significantly higher than those in control group, the differences were statistically significant（all P〈0.05）. 4Satisfaction of patients in observation group was significantly higher than that in control group, the difference was statistically significant（95.3% vs 83.7%, P〈0.05）. Conclusion Comprehensive nursing intervention for patients with lower limb fractures can reduce the incidence of deep venous thrombosis, shorten days of hospitalization, enhance compliance and nursing satisfaction. It is worthy of clinical application.
